She came in days later after the shot and told us of the reaction and showed us a picture. It was raised, red, swollen welt on her left arm. I told her to contact md and get his opinion. He still wants her to get shot next year but the regular flu shot. Patient's arm hashealed and the issue is resolved.

The patient was administered the Fluzone HD Quad 23-24 vaccine via PFS using a fliplock 25gx1" needle. Patient reported and verified NKDA or autoimmune disorders. She is currently well and has not been exposed to COVID in the last 14 days that she is aware of. Patient has received Fluzone HD in the past and has had no issues with any vaccines previously. Upon removal of the needle, the patient began to bleed slightly. This was cleaned using an alcohol swab. Immediately, a knot/bump began to develop. The patient was monitored for several minutes prior to allowing them to shop in the store. By the time the patient left, the "bump" had started to flatten. Patient is a retired nurse. She was informed I would be submitting a VARES report. I recommended she consider taking some ibuprofen or APAP as directed. Additionally, I suggested she try alternating 15min of heat and ice if needed for pain and swelling. I plan to further follow-up with the patient this afternoon. Patient was informed to contact her provider or EMS if symptoms worsened.

Patient reported cellulitis due flu vaccine received on 11/2/2; she was calling to about the status of her antibiotic and triamcinolone 0.1% cream written on 11/7/23. The rph asked the patient if she had if she had reported this matter to the pharmacy before, but she said that she hadn't and didn't blame the pharmacy/immunizing RPH for the adverse event because she said that "cellulitis can happen". She said she felt the issue start "immediately, right away after getting the vaccine" but did not say anything to the pharmacy staff at the time.

This case involves 69 years old female patient who had administration of inadequate dose as they screwed on the needle tight but when they administered, a good amount squirted out the space where the needle was attached with no reported adverse event while receiving influenza quadrival A-B high dose HV vaccine [Fluzone High-Dose Quadrivalent]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 29-Sep-2023, the patient received a total dose of suspect influenza quadrival A-B high dose HV vaccine (formulation, strength, dosage- not reported) (lot U8132DA, expiry date- 01-JUN-2024) via unknown route in unknown administration site for Immunization. On 29-SEP-2023 the patient had administration of inadequate dose as they screwed on the needle tight but when they administered, a good amount squirted out the space where the needle was attached with no reported adverse event (incorrect dose administered) (latency- same day) following the administration of influenza quadrival A-B high dose HV vaccine. Reportedly, Pharmacist wants to know if there are any guidelines regarding revaccination FLUZONE HD QIV Action taken was not applicable. At time of reporting, the outcome was Unknown for the event. Patient was asked name, DOB and age and they responded with name, 8/15/XX and 65, was asked a second time about age and confirmed 65. NP administered the high dose flu vaccine, based on patient report of age and did not confirm age in chart. Miscommunication about age and birth year led to error.
