She received both Fluzone and Comirnaty on her left deltoid and described that after receiving Fluzone vaccine, she immediately experienced pain and locked shoulder. on 11/18 patient had swelling at the injection site 30 minutes after vaccination while she was in the store. she proceeded home and continued having other reactions such as tiredness, body aches, itchy skin and eyes, hot lips and mouth and swelling continued to increase to up to 4 inches at the injection site. on 11/19 at 3am patient reported taking Benadryl and at 2:30pm when reaction was reported to us she said symptoms had subsided

Pt described nerve pain going in and down the right arm that went away within the hour of getting the vaccine. Four hours later, pt calls to report vision problems, seeing double ("up and down crown, double vision") that lasted for about 25 minutes and resolved. Pt then reports lower back pain (level 2-3) around 6:45pm, neither sharp nor dull pain.

mistakenly given fluzone hd with no reported adverse event; after the vaccine had been administered that she already received a flu shot from her doctor with no reported adverse event; Initial information received from country on 10-Nov-2023 regarding an unsolicited valid non-serious case received from a pharmacist. This case involves Elderly female patient who was mistakenly given Influenza Quadrival A-B High Dose Hv Vaccine [Fluzone High-Dose Quadrivalent] with no reported adverse event as the patient came in to get COVID, Pneumonia and RSV vaccines and after the Vaccine had been administered that she already received a flu shot from her doctor The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On an unknown date, the patient received a dose of 0.7 ml of suspect Influenza Quadrival A-B High Dose Hv Vaccine (lot: U8138AA and expiry date: 30-Jun-2023) (strength and formulation unknown) in unknown administration site for Immunization along with co- suspect vaccines Pnemonia, RSV vaccine and COVID-19 vaccine not produced by Sanofi Pasteur (strength, batch number, formulation, expiry date unknown) via unknown route in unknown administration site for prophylactic vaccination (Immunization) . On an unknown date the patient developed a non-serious mistakenly given fluzone hd with no reported adverse event (wrong product administered) and after the vaccine had been administered that she already received a flu shot from her doctor with no reported adverse event (extra dose administered) (unknown latency). It was reported that Caller stated that the patient was mistakenly given FLUZONE HD even though the technician verified which vaccines were being given and patient agreed. Caller stated that the patient stated after the vaccine had been administered that she already received a flu shot from her doctor on Tuesday, 07-Nov-2023 so patient has now received two flu vaccines. Pharmacist wants to know if there is anything that they should watch out for. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive and Module of the Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

Technician confirmed with her that she wanted the flu shot and continued to process it. Insurance did NOT give any hard halt and vaccine was processed through her insurance. Our vaccinating pharmacy technician accidentally administered a fluzone high dose flu shot to a 59 year old patient at an off-site vaccine clinic. I am submitting this report to document the incident. The patient has not reported any adverse events. I contacted the assisted living facility where we were when the vaccine was administered to inform them of the error since the patient has memory problems.
