Symptomtext

Patient received two vaccines. Prior to the vaccinations, Pt underwent a fingerstick. Pt commented on the amount of blood, but did not state she was feeling unwell. The first vaccine, the flu shot, was administered in the right arm with no complications. The second vaccine, the covid vaccine, was administered in the left arm at 10:15am. The medical assistant turned slightly to retrieve a bandaid when the Pt began to lean forward on the exam table. MA asked if she was alright, but Pt suddenly fell forward off of the exam table. MA tried to grab her left arm, but was unable to. MA was unable to catch the patient. Pt fell and hit the right side of her forehead on the ground. MA immediately notified and brought a doctor. Pt appeared to have a syncopal event presenting with stiff limbs. She was also bleeding from a cut on her right eyebrow. Oxygen was placed by mask at 2 liters. Pt was unconscious for less than 1 minute. When she awoke, she appeared alert and returned to baseline cognitive functioning. Heart rate and respiratory rate were normal with oxygen saturation at 98%. Pt sat up and was given apple juice and crackers. Pt was monitored by a nurse and the doctor for 1 hour and remained sitting on the floor. Pt stood up after an hour to sit on exam table. Once she was on the exam table, she had a near syncopal event, and paramedics were called. She again returned to her normal baseline cognitive state and again heart rate, pulse ox, and respiratory rate were normal. Patient was transported to hospital by EMS.

PT STARTED TO FEEL DIZZY, ALMOST FAINTED, MOM WAS ABLE TO CATCH PT. PT WAS ABLE TO FEEL BETTER AFTER DRINKING WATER AND EATING CHIPS. PT HAS HAD THIS HAPPEN BEFORE, 5 YEARS AGO WITH ANOTHER VACCINE PER MOM AT AGE 11. MOM REPORTS THAT THIS ALSO HAPPENS TO HER AFTER VACCINES AND L AB DRAWS.

bad taste in my mouth; itchiness on my left breast; Initial information received on 14-Nov-2023 regarding an unsolicited valid non-serious case received from a consumer/non-healthcare professional. This case involves a 47 years old female patient who experienced bad taste in mouth and itchiness on left breast after receiving vaccine INFLUENZA QUADRIVAL A-B VACCINE [FLUZONE QUADRIVALENT]. The patient's past medical treatment(s), vaccination(s) and family history were not provided. The patient had no medical history, concomitant disease or risk factor. On 20-Oct-2023, the patient received unknown dose of suspect INFLUENZA QUADRIVAL A-B VACCINE (Suspension for injection) (lot U8201FA) via intramuscular route in the left deltoid for Annual vaccine to prevent flu (Influenza). On 23-Oct-2023, the patient developed bad taste in my mouth (taste disorder) and itchiness on my left breast (pruritus) 3 days following the administration of INFLUENZA QUADRIVAL A-B VACCINE. Patient did not experience any additional symptoms/events. There were no lab data/results available. Action taken: Not applicable. It was not reported if the patient received a corrective treatment for all the events. At time of reporting, the outcome was Not Recovered / Not Resolved for both the events.

diagnosed with COVID-19; This spontaneous case was reported by a patient and describes the occurrence of COVID-19 (diagnosed with COVID-19) in a 61-year-old male patient who received mRNA-1273 BIVALENT .222 (MODERNA COVID-19 VACCINE, BIVALENT (ORIGINAL AND OMICRON BA.4/BA.5)) (batch no. 053D22A) for COVID-19 prophylaxis. Co-suspect product included non-company product INFLUENZA VACCINE INACT SPLIT 4V (FLUZONE QUADRIVALENT) for an unknown indication. Previously administered products included for COVID-19 prophylaxis: Moderna (Dose:1, Lot number: 037B21A, dose: 0.5 ml, site: Left arm) on 12-Apr-2021, Moderna (Dose:2, Lot number: 022021A, dose: 0.5 ml, site: Left arm) on 11-May-2021, Moderna (Dose:3, Lot number: 069H21A, dose: 0.5 ml and site: Left arm) on 20-Dec-2021. Past adverse reactions to the above products included No adverse event with Moderna, Moderna and Moderna. Concomitant products included OMEPRAZOLE for GERD, ROSUVASTATIN for Hyperlipidemia. On 14-Oct-2022 at 5:15 AM, the patient received fourth dose of mRNA-1273 BIVALENT .222 (MODERNA COVID-19 VACCINE, BIVALENT (ORIGINAL AND OMICRON BA.4/BA.5)) (unknown route) .5 milliliter. On an unknown date, the patient received dose of INFLUENZA VACCINE INACT SPLIT 4V (FLUZONE QUADRIVALENT) (unknown route) 1 dosage form. On 16-Nov-2023, after starting mRNA-1273 BIVALENT .222 (MODERNA COVID-19 VACCINE, BIVALENT (ORIGINAL AND OMICRON BA.4/BA.5)), the patient experienced COVID-19 (diagnosed with COVID-19). At the time of the report, COVID-19 (diagnosed with COVID-19) outcome was unknown. DIAGNOSTIC RESULTS (normal ranges are provided in parenthesis if available): On 16-Nov-2023, SARS-CoV-2 test: Positive. No treatment medications were provided.
