Symptomtext
Patient arrived at vaccination tent to receive influenza vaccination on 11/28/22. Administered Fluzone 0.7mL to patients right deltoid at 12:20pm. After removing flu vaccination from patients arm, patient reports the nurse expelled fluid onto the ground before discarding syringe into sharps container. The patient did not convey his concerns to anyone at the time, but after leaving, escalated his concerns. Upon further investigation, it was determined that patient did not receive entire dose of Fluzone Vaccination, and per instructions, patient was contacted and offered re-vaccination with FluZone.
