Patient presented to clinic for High Dose Flu Vaccine that was adminstered in the Right Deltoid. When documenting adminsistration in the patient's chart, it was discovered that the patient had also received a High Dose Flu Vaccine on 10/24/22. This was reviewed with the ordering provider and it was determined that this would not be harmful to the patinet. When this was discussed with the patient, she was adamant that she had not received the previous High Dose Flu Vaccine on 10/24/22 despite there being documented administration and a signed flu consent in the patient's chart on 10/24/22. Patinet was stable with no side effects and no adverse effects at the time of leaving the clinic more than 30 minutes later.
