Before administering the vaccine, I asked the patient to provide their date of birth and age so I could write it on their form. She gave date of birth (which ended up being different than the DOB I got from insurance). I asked her how old she was, and she said she was 66. I wrote both down on her consent form and prepared the Fluzone high dose because she said she was 65+. I administered the vaccine and the error was identified when billing insurance today, 10/31/2022.
