Symptomtext
The MA who administered the dose did not combine (reconstitute) the two vials. She administered only the DTaP-IPV and not the Hib. She self-reported, notified the provider and the family. An incident report was done at work, signs were posted on the vaccine refrigerator regarding the need to mix the two vials and they are now bound together. An email was sent to all of the managers at the other clinics to please do the same quality updates.
