ER visit for seizure activity that occurred about 845am on 4/16/21. Diagnosed with febrile seizure. Temperature in ER was 101.3 rectal pulse ox 99% on room air. He was treated with Tylenol and ibuprofen and observed. He was discharged about 12:28pm after no further seizure activity.

05/07/2021 phone message. 16:41: Spoke with pts mother who states pt did not have a restful night last night and today just wants to sleep and not taking formula well. She states he ate a couple ounces this morning (he normally has a 5oz bottle every 3-4 hours) - he has only had a couple ounces today and wants to sleep. She denies any fever, runny nose, cough. She states he seems to be spitting up what she is feeding him. Patient Admitted on 05/08/2021 for intussusception

TECHNIQUE: Ultrasound multiplanar gray-scale and color Doppler images of the abdomen were obtained using a 9MHz linear transducer. The exam is limited to exclusive evaluation of the bowel. FINDINGS: The terminal ileum is visualized telescoping into the cecum and ascending colon to the level of the liver. The intussusception measures up to 2.7 cm. There is moderate volume free fluid throughout the abdomen and pelvis. The appendix is identified and is partially pulled up into the intussusception. The appendiceal tip is unremarkable. IMPRESSION: Right upper quadrant ileocolonic intussusception with moderate volume abdominopelvic free fluid.
