On 12/09/2020 Patient reported to her school Nurse where she works that she has suffered a shoulder injury from a vaccination on 10/27/2020. On 12/10/2020 patient reported she still had shoulder pain and difficulty moving it. Reported an appointment had been made with Dr. for 12/17/2020. On 12/22/20 we f/u about appointment and patient reported "I do not yet have a clear treatment plan, but it is clear that all of the structures of the rotator cuff are involved." Patient reported she is awaiting approval for an MRI.

Symptomtext

Swelling of left arm, left leg and right leg. Sore joint in left hand. Hand swelling lasted for over 12 hours. Swelling in legs lasted for up to 5 days. Headache that lasted 4-5 hours. Treatment: took Benadryl and Advil.

Symptomtext

Pt received vaccines and walked out to waiting area. Stated to her friend that she didnt feel well so she sat down in the chair shortly after sitting pt began to stiffen backwards in the chair and turn bluish color of face and lips. Very shallow and quick breathing. Pt lost bladder control and her right arm stiffened straight down.

The biopsy 4/19/21 shows "naked" granulomas, as are typically seen in sarcoidosis. In some of the deeper sections, there is foreign material within multinucleated giant cells, suggesting that this might be a reaction to the egg free flu vaccine noted in the patient's clinical history. Special stains for organisms are all negative

administer an expired Flublok with no AE; Initial information regarding an unsolicited valid non-serious case was received from other health care professional via Medical Information (GMI) (Reference number- 00530251). This case involves a adult female patient who was administered an expired dose of INFLUENZA QUADRIVALENT RECOMBINANT VACCINE [FLUBLOK QIV] (lot UJ446AA, expiry date: 18-Mar-2021) via unknown route in unknown administration site for prophylactic vaccination on 22-Mar-2021 (expired product administered). Medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included PNEUMOCOCCAL VACCINE POLYSACCH 23V (PNEUMOVAX-23) for Immunisation. It was a case of an actual medication error due to expired vaccine used (latency same day). At the time of reporting, patient had no adverse event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
