Symptomtext

Per parent, client developed fever of either 100.5 or 105 degrees. Parent was unsure. Parent states they witnessed convulsive like activity. Parents took child to Hospital ER. Per parent, ER gave child Tylenol. After fever decreased and they were discharged approximately 2 hours after arrival. Today child is not very active. The only other symptom is slight local swelling of thighs.

Angiogram pulmonary normal Pallor Rash Rash erythematous Rash papular

Symptomtext

Five (5) minutes after receiving vaccines, patient started with a facial rash. Patient with erythematous, blanching, papular eruptions over face and upper chest. No hives. Lungs CTA, no increased respiratory effort. Observed in office x 20 minutes with spontaneous fading of the rash. Family was called 3 hours after discharge to follow up and parents had reported that the rash had completely resolved without any further invention (ie: oral or topical medications)

7/14/22 1:22PM Pt mother called is stating pt recently got vaccines on L arm on 7/11/22 the next morning her arm looked like a bug bite and today this morning pt was itchy on her arm and now has red spots were injection was placed , mom tried putting bendayrl cream and zytrec med but is not helping and is getting worse 7/14/22 2:43PM mom - she states child got vaccines on 7/11- area of concern is L arm - Confirmed that she was given the DTAP on the L arm. Mom states on 12th are was red and a bit swollen- looked like a bee sting- she applied benadryl cream and gave her zyrtec. Today she woke up and the area looks like a large square (red) and is swollen are warm to touch. It is also really itchy. Scheduled at 5pm - also asked mom to email me a photo pre appt. Mom agrees. Patient missed appointment on 7/14/22 at 5PM. 7/18/22 9:04AM : Spoke with Mother, and pt was seen at an urgent care 7/14/22 for her concerns, per mother she was advised to continue the use of Benadryl for 24 hours. Mother reports today that patients arm is clearing up and mother had no further concerns at this time. Mother declined offer to schedule a f/u appt with out office. Mother advised that a VAERS report would be filed, due to the adverse reaction patient had. Mother had no further questions at this time.
