Symptomtext

The night the vaccines were administered (8/16/22( patient started complaining of "heart pain." Mother took patients temperature was 100.0 F. Patient was given Tylenol Q4H until patient was seen in office on 8/17/22 around 9 am. Initial presentation in office was tachycardia (140-150) and a fever of 99.8. Patient was given 7.5 mL of Tylenol in office. After 30 minutes patient heart rate reduced to 120 BPM. Patient tolerated medication in office and effectively had HR reduced to a normal range. Patient instructed to follow-up in ER if symptoms returned.

Symptomtext

Pt presents to clinic with a mild cough and h/o asthma. Pentacel was administered instead of Hib forecasted. Upon speaking with parent, he reports patient experienced off and on fever. Parents continued giving child Tylenol when needed. Pt is feeling "much better" today.

I accidently administered Pentacel that had expired two days prior to administration. It expired 04-02-2023. I administered it on 04-04-2023. No adverse events or reactions noted.

expired ACT-HIB vaccine administered to 2 patients with no reported adverse event; Initial information received on 14-Apr-2023 regarding an unsolicited valid non-serious case received from a healthcare professional. This case involves an unknown age and unknown gender patients who had administered expired haemophilus type B (HIB) (PRP/T) vaccine [ACT-HIB] with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On an unknown date, the patients received a standard expired dose of suspect haemophilus type B (HIB) (PRP/T) vaccine Powder and solvent for solution for injection at a dose of 0.5 ml total (lot UJ644AA, expiry date- 02 -Apr-2023) via unknown route in unknown administration site for Immunization with no reported adverse event (expired product administered) (latency- same day). It was reported, The nurse can only provide lot number and expiration date and has no information regarding the patients and the health care provider. Nurse was also confirming regarding re-vaccination. Action taken was not applicable. At time of reporting, the outcome was Unknown for the event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
