Client traveling and interested in YF vaccine. Discussed client's PMH, medications, allergies (including benadryl), past throat swelling reaction with Hep B and Covid vaccines, increased risk with YF vaccine over 60 years of age and option of Medical Deferment Letter. Client chose to receive the YF vaccine and signed consent. 9:47 - YF vaccine given. Author remained with the client to monitor symptoms and continue with consultation. 9:51 - Client started coughing, difficulty catching her breath, facial redness 9:52 - Epinepherine 0.3 ml given IM per anaphylactic protocol. 9:54-Continue to monitor the client. Worsening cough, shortness of breath, weak voice and difficulty talking. 9:55 - Call to EMS. Continue to monitor the client. Ambulance in route. 10:01- Worsening cough, client states tongue feeling "thick and swollen", client voice very weak and talking triggers coughing. Alert and oriented x 3. RR 18, radial pulse 93. 10:02 - Epinepherine 0.3 ml given IM (second dose) 10:03 - EMS at client's side performing vitals. BP 142/90. HR 98. RR 22. Client Alert and oriented x3. Visible shaking in the hands. Client gave verbal consent for transport to Hospital. Client denied wanting anyone called for her. Card given to client. Staff will follow up with the client within 24 hours to check on her status. Invoice left open at this time. 10:07 - Client left with EMS via transport to Hospital Alert and oriented x 3, VSS, ambulated to ambulance.
