10/4/2021 - Sudden stabbing pain in upper right chest. Radiates to neck and back, right side 10/5-10/6 - pain subsides, but coughing 10/7 - Stabbing pain in upper right chest more severe, scheduled appointment with PCP 10/7 - PCP (Primary Care Physician) did EKG, D-Dimer and Chest X-ray. D-Dimer positive; X-ray shows fluid in bottom of right lung. PCP orders CAT scan 10/8 CAT scan shows pulmonary embolism. EKG and other heart health tests come back normal. Sent home with 20mg/day apixaban (blood thinner). NOTE: no history of clotting, no risk factors for clotting.

Symptomtext

Patient experiencing arm pain both during movement and at rest, worsening during movement until raising arm to shulder height, at which point pain becomes unbearable. Patient has tried acetaminophen and ice, without relief. Patient expereincing some level of relief with 400mg of ibuprofen every 6 to 8 hours, but not complete relief. Patient has been refered to primary care physician for evaluation.

Vaccine received on 8/22/21. Began experiencing pins and needles sensation in hands and feet within 24 hours. Four days later, began experiencing tremors. On 09/01/2021, began experiencing muscle weakness, trouble walking, and slurred speach, and was hospitalized on same day. Discharged from hopspital on 9/3/2021.

Symptomtext

Patient showed me a picture of her arm the night after the shot. It was a bit swollen with some red patches from her shoulder down the arm to the elbow. She said it was swollen, hot, red, and painful. She did not touch it because it was painful even without contact. The reaction started noticeably the night after the shot and lasted at least 3 days.

Patient received his yearly influenza vaccine at one of our school based clinics. His reaction started about 2 hours after receiving the vaccine. He started with a rash around the injection site area which grew to his legs, arms, abdomen, neck and face. He also had welts that were very itchy.

Symptomtext

Patient stated that approximately one hour after receiving her Fluzone Quad immunization today she was experiencing a local reaction that consisted of raised bumps (potentially hives). There was minor urticaria associated with the reaction as well. Patient was instructed to take Benadryl and/or Advil to alleviate the reaction along with a cold pack to soothe the area. Patient was also instructed to inform their Primary Care Provider or seek emergency treatment if the condition spread or worsened. Patient expressed understanding and was satisfied at the time with this solution. No further information/action has been presented at this time.

pregnant patient receive the vaccine/ no AE; Initial information regarding an unsolicited valid non-serious case was received on 31-Aug-2021 from a Physician via Medical Information (Reference number- 00748401) and transmitted to Sanofi on 31-Aug-2021. This case involves a 35-year-old female patient who had taken INFLUENZA QUADRIVAL A-B VACCINE [FLUZONE QUADRIVALENT] while she was pregnant (Maternal exposure during pregnancy) with no adverse event. some of the dose did not go into the patient's arm and leaked out, no AE; Initial information regarding an unsolicited valid non-serious case was received from pharmacist (Reference number- 00715961) and transmitted on 09-Aug-2021. This case involves a 54-year-old female patient who received INFLUENZA QUADRIVAL A-B VACCINE [FLUZONE QUADRIVALENT] from which some of the dose did not go into the patient's arm and leaked out (underdose). The patient's medical history, past medical treatment(s), vaccination(s), concomitant medications and family history were not provided. On 09-Aug-2021, the patient received 0.5 mL dose of suspect INFLUENZA QUADRIVAL A-B VACCINE (lot UJ686AA and expiry date: 30-Jun-2022) via an intramuscular route in the arm for prophylactic vaccination from which some of the dose did not go into the patient's arm and leaked out (underdose). It was reported that "Pharmacist wanted to know what the protocol should be." This was a case of actual medication error due to vaccine underdose (latency same day). At time of reporting, no adverse events were reported. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
