Patient came to pharmacy with parents to receive both pfizer 5-11 covid vaccine as well as the flu vaccine. The patient received covid vaccine in his left arm and was fine but the once received his fluzone vaccine in the right arm he fainted. We believe it was from looking at the needle while being injected. His breathing was ok, we lied him flat opened airway and used ice pack on forehead. He continued to seem dazed, so we called 911 to have him looked at. The paramedics decided it best to transport to the ER. There the doctors felt it was a vasovagel response to getting the second vaccine, they did not feel it was related to the vaccine itself. He was sent home from er and cleared to participate in his sports activities that day.

Patient past out after receiving Fluzone quadrivalent vaccine and hit his chin on the floor. He lost consciousness and cut his chin on floor. Patient regained consciousness after a few minutes and felt sick and nauseous. Paramedics arrived at scene within about 10 -15 minutes while patient sat. Patient was evaluated by paramedics and Parents chose not to have patient go to the hospital with paramedics as his symptoms improved.

Approximately 5 minutes after receiving her covid vaccine, the patient was feeling unwell, dizzy, and lightheaded. She then lost consciousness for 30-60 seconds. The patient was assisted into a laying position on the floor and was provided with a pillow, ice packs, and a water bottle. The patient continued to improve over the next 10 minutes and we allowed her to sit on the floor while leaning against a wall. As she continued to improve, we allowed her to sit in a chair but asked that they stay in the pharmacy area a little longer for observation. Within 5 minutes of this the patient lost consciousness again and did experience some muscle rigidity, which was short-lasting, and the patient was assisted to the ground and the same interventions taken. The patient left in an ambulance out of an abundance of caution but required no further medical intervention. The patient is at home and well per a phone conversation later in the evening.

About 15 mins after Echo Tech at the hospital, working when received the vaccine received her vaccine she returned and said the sight was itching, I assessed the sight and it was red, raised, swollen, and irritated looking. I removed the band aid and cleaned the sight with alcohol. She said she wasn't having any respiratory or cardiac symptoms. Her vital signs were stable. I told her I thought it might be the band aid and to let it air out and I would check on her in 30 mins, I informed her she could be seen at the clinic or if she experienced any shortness of breath or cardiac symptoms to go to the ER. When I went to check on her 30 mins later, the sight was no longer red, but was edematous and hot. She said she had a few cardiac palpitations. I asked if she want to go to the ER, she said no. I told her to take Motrin now and Benadryl when she got home. Also she could put a cold pack on if it became too uncomfortable.

when administering vaccine "little to none" was actually injected with no adverse event; not attached properly to the prefilled syringe with no adverse event; Initial information was received on 01-Nov-2021 regarding an unsolicited valid non-serious case received from a pharmacist and non-healthcare professional via the Regulatory Authority (Regulatory Authority Number - 00836403). This case involves a 61-years old male patient who was administered with suspect INFLUENZA QUADRIVAL A-B VACCINE [FLUZONE QUADRIVALENT] had little to none was actually injected due to needle not attached properly to the prefilled syringe with no adverse event (Incorrect dose administered) (needle issue). The patient's medical history, medical treatment(s), vaccination(s) and family history were not provided. No other concomitant vaccines were not provided. On 31-Oct-2021, the patient received 0.5ml dose of suspect INFLUENZA QUADRIVAL A-B VACCINE (lot number UJ687AA and expiry date not reported) via intramuscular route in the left deltoid for prophylactic vaccination. It was a case of an actual medication error due to inappropriate dose of vaccine administered and needle issue (latency was on same day). At the time of report, no adverse event reported. Events outcome was unknown. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

Parents signed child up for flu shot at enrollment. Did not tell anyone that he had been given shot on 9/29/21. Child was given vaccine he had been signed up for due to miscommunication. Parent informed school this AM that he had been given the shot in September. Child has no adverse reactions to report.

Patient stated that her left arm starting feeling hot on Sat, Oct 23rd in the afternoon. When she woke up on Sun, Oct 24rd there was blistering of the skin and it was very red. She went to urgent care immediately and got treated for the reaction.

The child inadvertently was given Pediarix and Pentacel when Pediarix and HIB were intended therefore the child received a double dose of Dtap and IPV. At the time of this filing the child has not experienced any signs or symptoms related to this error.

Pt was administered the Fluzone Quad flu shot at around 7 pm tonight. About 5-10 min later patient approached the pharmacy saying that he was wheezing and felt like his throat was swelling. I took patient back into the immunization booth and had him sit down. He I had a technician page code white while I got an epi-pen. I administered the Epi-pen and had my technician call 911. I asked the patient if he thought he could swallow benadryl liquid and he said yes, so I gave him 50 mg of benadryl liquid and then gave him a water to sip. I checked his blood pressure every 5 minutes until EMS arrived to take over. EMS ran a EKG, listened to his breathing an checked blood pressure. They asked pt if he wanted to go to the hospital and pt declined. Pt stayed at the pharmacy for another hour while we frequently monitored his blood pressure and pulse ox. Pt was starting to feel better at that time so went home.
