Symptomtext

**Administration Error** Patient stated that she was not feeling well and that she received a large knot and rash at the injection site. Patient went to the ER on 8/16/2021. Patient was told that she received a staph infection where the vaccination was inserted. Patient stated that pharmacist did not clean the site before inserting the injection. Patient stated that she has long red streaks of red going down her left arm. Patient is very dizzy and feels Nauseated. She also has a headache and lightheaded.
