PATIENT REPORTED LEFT ARM (PNEUMOVAX) WAS VERY SORE TO THE TOUCH AND HE WAS UNABLE TO LIFT THE ARM. I ENCOURAGED HIM TO APPLY A COOL COMPRESS TO THE SITE OF INJECTION AND SEEK MEDICAL ATTENTION IF IT DIDN'T IMPROVE WITHIN A FEW HOURS OR WORSENED.

Patient was administered a booster Pfizer vaccine and a FluHD both in right arm due to a mastectomy on her left breast. The doses were administered 1 inch apart in her right deltoid muscle. The patient was fine initially but when she was out sitting for her 15 minute interval she began having low back spasms, was having difficulty breathing and was feeling flush. 911 was called as well as Code White. I sat with the patient and got her a bottle of water. By the time paramedics arrived her breathing had returned to normal and the flushing in her face went down and the back spasms subsided. The paramedics took her blood pressure and oxygen level. The patient did not want to go to the hospital and left the store on her own.

Pt reported rash on her left left arm within 24 hours of Fluzone HD administration (10/12). She took one dose of cetirizine for some improvement, but presented at pharmacy 10/14 with distinct erythematous patches on her left arm, near and below injection site. No reports of itching or pain. No chest tightness or wheezing according to patient. I encouraged her to monitor and continue cetirizine daily and I would follow up with her on the weekend. Left a message with her on Sunday to follow-up, she returned phone call today, 10/20/21. Redness is still there, but looks better than onset. She mentioned she had a non-related PCP visit earlier this week, and PCP was not concerned.

after fluzone HD patient had minor itching and redness after vaccine administered at site of injection, patient thought it could be from bandaid so we took it off. she stayed additional time in the pharmacy and monitored and she felt fine with no additional complaints. patient came in today (2 weeks later) and said the following tuesday after vaccine she went to the dr for something else and she reported the dr said the arm was red and swollen. today patient lifted up her sleeve and showed me where she believed I gave vaccine, asking if another dose would be needed. I showed the patient where her deltoid muscle is and that the vaccine was given in the deltoid. there was also a dark bruise about the size of a fist further down her arm, not near the injection site. she said the vaccine caused this and I asked when it appeared. the patient stated it showed up after she had a blood draw that tuesday on the same arm. I said clinically the bruise could be from that and she should have her dr look at it. she said she wasnt worried or upset, but wanted to let me know incase i could learn anything from it.

This case involves a 69-year-old female patient who experienced she looked a cooked lobster (she turned red) (erythema), metallic taste at the back of her tongue (dysgeusia) and she felt weak, she felt off for the entire rest of the day (asthenia) and sore arm (pain in extremity), after receiving INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE [FLUZONE HIGH-DOSE QUADRIVALENT]. The patient's medical history, past medical treatment(s), vaccination(s), concomitant medication and family history were not provided. On 30-Sep-2021, the patient received a dose of suspect INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE (lot UJ702AB, expiry date unknown) via an unknown route at an unknown administration site for prophylactic vaccination. On 30-Sep-2021 the patient developed non-serious she looked a cooked lobster (she turned red) (erythema), metallic taste at the back of her tongue (dysgeusia) and she felt weak, she felt off for the entire rest of the day (asthenia) (on the same day) following the administration of INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE. Patient called and left voicemail on 8/26/21 stating she was experiencing an unusual reaction to her flu shot that she received on 8/24/21. The pharmacist on duty called the patient back and went over what was happening. The patient stated that the night she received her vaccine, "a purple waterfilled raised area" appeared on the arm her vaccination was administered on. This was not at the site of the immunization but further down on her arm. Her arm became swollen and red to the touch. She did not seem to think she had a fever but stated she did not feel well. The purple raised area turned white today. The patient stated she has mycosis fungoid. The pharmacist advised her to go to urgent care sooner than later and follow up with her dermatologist.

I went to give this man a High dose Fluzone in left deltoid and when I stuck the needle in his arm the vaccine vial broke away from the needle and the vaccine squirted all over my gloved hand. It broke away from the needle at the part where the needle screws into the vial and this round piece also fell off. I pulled the needle immediately out of his arm and re-administered vaccine with new vial and different needle.

This case involves a 68 year old male patient who received two shots of INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE [FLUZONE HIGH-DOSE QUADRIVALENT] in the same day (extra dose administered). The patient's medical history, medical treatment, vaccination and family history were not provided. Concomitant medications included Varicella Zoster Vaccine Rge (Cho) (Shingrix) for Immunisation. It was a case of actual medication error due to extra dose administered (latency on same day). It was reported "Pharmacist reports consumer got two shots of Fluzone HD in the same day. He received one injection at the HCP (health care professional) office and one at pharmacy about a half an hour later. He denied he had gotten the Fluzone HD, however, his HCP office confirmed. She asks for any information regarding. Additional Description of event adverse events: Pharmacist reports consumer was given Fluzone HD at pharmacy and then it was confirmed that he had received it 30 minutes prior at HCP office". At the time of report no adverse event was reported. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

we had a vaccine event in our pharmacy today, and pt stopped by that table in the front of the store and received the flu hd shot there, then came to the pharmacy asking for a pneuomonia vaccine, our hand held programmer showed pt was pending both vaccines and he was inadvertently given the flu hd vaccine again.

Patient called about 2 hours after Fluzone HD was given. Said he thought he was having an adverse effect after the vaccine. Said he had right sided leg weakness. He does have trouble walking normally and uses a cane but usually it is his left leg. Said he was leaving the store after shopping around for a bit after the shot and though his right leg was weaker than normal. He was just relaxing and asked what he should do. Told him to rest for a bit and call back in an hour or so to let me know if there was any improvement or if it got worse. He never called back. I reached out to him around 4pm and left a message for him to call me back as I was checking on him and no response. Followed up the following day at 3:30 and still not able to reach him.

On 9/10/2021 patient came to the pharmacy seeking her flu shot. Flu shot was prepared for the patient. The vaccine administrator called out a different patients name to which the patient responded and confirmed she was the other patient verbally. When asked if she was here for her second dose of pfizer covid vaccine she again responded yes. The patient received the pfizer vaccine that was intended for a different patient. The incident was discovered when the intended patient asked on the status of her vaccine.
