Dizziness Presyncope

Symptomtext

Approximately 3-4 minutes after receiving his vaccinations patient became distressed watching his sister getting ready for her vaccines. He became vaso-vagal and slid down to the floor. He regained conciousness within 5 seconds. Vitals were stable. He was monitored for approximately 30 minutes after. He became dizzy again when his sister got his shots. He recovered within 30 minutes and went home with parents.

Symptomtext

Child presented with BL thigh redness and itching . O/E - comfortable child, no respiratory distress. the left thigh -swelling noted with erythema and tenderness- mild over the injection site . urticaria within the erythema . The entire upper thigh was affected. The left thigh showed erythema with wheal and flare . the left cheek had erythema also. Pt itching all 3 areas . applied cool pack as well as given 5 ml Diphenhydramine po . Within 30 mins of observation, all swelling and erythema were significantly decreased. Pt was Dced home with q6 Benadryl, cool compress to area and instructions to go to ER if worse, difficulty breathing or appearance of cough. Allergy referral done and mom took pictures of the initial reaction on her phone

Patient received an extra dose of flu vaccine. She received a dose of Fluzone on 04/23/22 despite having a previous dose on 12/09/21 and her initial 2 dose series during the previous flu season on 03/04/21 and 04/05/21. This information was not available at the time patient received 04/23/22 dose.

Symptomtext

This nurse gave a flu shot to the patient using system information that was not the most recent. Record nurse received was blank (name misspelled). Patient stated she had a copy of her record. Patient also stated her school wanted certain shots completed for registration. Pt reported receiving a flu shot last fall before surgery but had no documentation. This shot was not listed on her report. This nurse advised patient that flu shots are the only vaccination that can be accepted for reporting as a self-attestation, so she did not have to get a flu shot today but the date can be added to her record if she was sure she received it and knew the date. The patient was not completely sure and decided to get another flu shot, which was given. When her current report was accessed, the previous flu shot was listed. Pt was informed and monitored for an additional 20 minutes without incident.

Symptomtext

Pt came to clinic for care and requested Pneumonia and flu vaccines. Vaccines where order by provider and this nurse administer the vaccines to pt. Since the month on the expiration date for the pneumovax-23 vaccine was was abbreviated as Jan, it was mistaken for June. Vaccines where administer and this nurse went back to get the lot number and expiration date for documentation purpose. At the moment it was noticed that the actual expiration date for the pneumovax-23 vaccine was actually Jan 27,2022 not Jun 27,2022. The expiration date was verify with the ordering provider and at the time the pt. was notified of the administration of the expired vaccine. Pt was advise by the provider to return in a week to be revaccinated for the Pneumonia. Pt understood and agree to advise. Pt had stable vital signs and it look well over all. Pt was talking and showing no signs of distress or discomfort, or signs of adverse effects or reaction at time of discharge.

MA inserted needle for Flu vaccine. Child pulled back and had no success of injecting the liquid. Second time she poked child. She was able to get some medication when child pulled back. MOC asked to inject the child with vaccine, she told her she couldn't.
