10/25/2022: 11:58 AM: Flu shot administered in right shoulder, too high, had sharp pain. 9:00PM Extreme pain in Rt. shoulder and upper arm, could not sleep. 11/01/2022: Saw Dr.; started on steroid pack and Ibuprofen 800 Mg 3 x day. Steroid did not help 11/11/2022; Went back to Dr.: Had x-ray taken, wanted an MRI scheduled. Felt since this needed an MRI, I made appt with Dr., orthopedist. Dr. put me on light duty at work. No lifting of any kind rt. arm 11/16/2022; saw Dr., did another x-ray and scheduled MRI for next Monday. Gave me a prescription for the pain, dilaudid 2 mg. 1 tab at night to help me sleep. 11/21/2022: MRI done. 11/23/2022; To Dr., saw, his PA. Said there is a slight tear in the rotator cuff and severe myoedema. 11/29/2022; Saw Dr.; discussed the MRI, the severe myoedema and tears in muscle. Gave me a steroid shot. Placed on light duty with no lifting rt. arm for 6 wks. Told me to take ibuprofen800 mg 3x a day. 12/14/2022; steroid shot has worn off, more pain with active movement. Unable to raise arm to the side. 01/10/2023; Went back to Dr.; did not want to try another steroide shot; wants me to remain on light duty with weight restriction of less than 5 lbs. 01/29/2023; Off and on intense pain in right arm, unable to lift right arm past 85-90 degrees. 02/10/2023; still having pain with movement and still taking Ibuprofen 3x a day

Injection site pain

Symptomtext

Today is nine days after vaccination. Patient complained that she is still experiencing sharp pain from the injection site. She said that pain is the most intense at the site and less intense further away from the site. I asked about shoulder movement and stillness and she said she doesn't have that. We refer her to visit her PCP to further investigate the issue. She said she will do that and update us on what the doctor said.

Pt asked for a Flublok vaccine. I gave him a Fluzone HD. The boxes look similar, and the peel-off stickers look almost identical. I called him about 48 hours later and he said his only side effect was a "sore arm." No treatment.

AFTER FLUZONE HD VACCINATION, PT DEVELOPED ARM PAIN. HER PAIN WAS PROGRESSED TO ENTIRE LEFT ARM AND SHOULDER. PT COULDN'T SLEEP WELL. 9/27/22 MORNING PT CALLED AND CAME IN TO SHOW ME. HER ARM WAS TENDER TO TOUGH. INJECTION AREA IS CLEAR. NO SIGNS OF INFECTION. INJECTION SITE WAS AT DELTOID MUSCLE AND NOT TOO HIGH TO CLOSE TO SHOULDER. RECOMMENDED PT TO USE ICE PACK TO ICE IT LOCALLY. ALSO GAVE PT OTC ACETAMINOPHEN FOR COMFORT. 9/28/22 I CALLED AND CHECKED WITH PT. PT SAID SHE FELT MUCH BETTER. SHE STILL FELT IT THOUGH. I SAW PT ON 9/29/22. SHE WAS OK..

patient inadvertently received a dose of fluzone high dose quadrivalent with no reported adverse event; Initial information received on 13-Dec-2022 regarding an unsolicited valid non-serious case received from other health professional. This case involves 63 years old male patient who inadvertently received a dose of influenza quadrival A-B high dose HV vaccine [Fluzone high-dose quadrivalent] with no reported adverse event. Concomitant medication included moderna bivalent booster. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 09-Dec-2022, the patient inadvertently received 0.7 ml dose of suspect influenza influenza quadrival A-B high dose HV vaccine frequency: once with (Strength = high dose) (lot UJ918AA and expiry date:30-Jun-2023) (form: unknown) via intramuscular route in the right deltoid for immunization, with no reported adverse event (product administered to patient of inappropriate age) (latency: same day). Action taken was not applicable. At time of reporting, the outcome was Unknown for the event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

fluzone high dose qiv was administered to at 15 year old patient instead of the prescribed fluzone qiv on 04oct2022 with no reported adverse event; Initial information received on 05-Oct-2022 regarding an unsolicited valid non-serious case received from a other health professional. This case involves a 15 years old female patient who stated that influenza quadrival A-B high dose hv vaccine [Fluzone high-dose quadrivalent] was administered instead of the prescribed fluzone qiv on 04oct2022 with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 04-Oct-2022, the patient received a dose of suspect influenza quadrival A-B high dose hv vaccine at a dose of 0.7 ml once (lot UJ918AA, expiry date - 30-Jun-2023) (with an unknown strength, form) via intramuscular route in the right arm for Immunization. On 04-Oct-2022 the 15 years old female patient who was administered influenza quadrival A-B high dose hv vaccine instead of the prescribed fluzone qiv with no reported adverse event (wrong product administered) (latency: same day) following the administration of vaccine. Action taken was not applicable. At time of reporting, the outcome was Unknown for the event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices.
