Patient was admitted to hospital with cardiac ischemia on the 29th of November. He subsequently coded and was taken to cath lab and found to have clot in RCA. Clot removed and stent placed. He then became unstable again and found to have clotted off his brand new stent. He was already taking eliquis and ASA prior to admit and was started on heparin gtt on admission. He was transferred to our facility on November 30th for cardiogenic shock with in stent re-thrombosis despite anticoagulation. He underwent repeat RCA PCI and impella LVAD device.

Discharge Provider: MD Primary Care Provider at Discharge: MD Admission Date: 9/11/2023 Discharge Date: 9/14/2023 DETAILS OF HOSPITAL STAY: PRESENTING PROBLEM: Hypokalemia [E87.6] Generalized weakness [R53.1] COVID-19 [U07.1] Admission H&P: Patient is a 94 y.o. female with prior medical history of AFib, CHF, GERD, hyperlipidemia, hypothyroidism and who presents today with weakness, fatigue and cough. The patient reports falling at home on Friday when she was getting up to take her morning dose of Synthroid she suddenly fell to the ground. She does not recall hitting her head or losing consciousness. She reports possible lightheadedness prior to falling and notes that she has been feeling unsteady since the fall. Denies any headache prior to fall. She does have a remote history of seizures but denies any recent seizure activity. Denies chest pain, or shortness a breath. She does have a nonproductive cough which started around Friday as well. She reports significant fatigue and weakness. She lives at home alone and typically independent with ADLs. She reports minimal orally intake over the past 3 days. Denies any fevers but does endorse chills. Has mild discomfort in her back and left side of the chest but has not required anything for pain . In the emergency department several scans were completed including CT cervical and thoracic spine which were both negative for acute process. CT thorax no evidence of trauma and unchanged small pericardial effusion. CT of the head was negative for acute process. Labs revealed a mild worsening of creatinine of 1.51 and potassium of 3.0. Potassium was replaced in the emergency department she was also found to be COVID-19 positive. An order for orthostatic vital signs and ambulatory pulse ox was placed however the patient was too weak to ambulate. Admit to hospitalist service for physical therapy occupational therapy and IV fluid resuscitation. HOSPITAL COURSE: 94 years old woman presented after a fall due to generalized muscle weakness. She was found to have COVID positive and acute kidney injury. AKI resolved with IV fluids and HCTZ 12.5 mg was discontinued on discharge since it worsened her kidney function. Her BP remained controlled on amlodipine 2.5 mg BID, so BP needs to be managed outpatient per PCP. PT and OT recommended discharge to home with home health care services. For COVID 19, no remdesivir was given was because of her AKI and reduced creatinine clearance. No paxlovid was given on discharge either due to low creatinine clearance per pharmacy. Patient was not requiring oxygen inpatient. Patient was discharged in hemodynamically and clinically stable state. For GERD, pantoprazole 20 mg daily was continued but famotidine dosing was adjusted to 20 mg every other day due to renal adjustment per pharmacy.

FAHRENHEIT; weakness; FLUZONE HIGH-DOSE QUADRIVALENT was inadvertently given to a patient who was under the age of 65; Initial information was received on 14-Nov-2022 regarding an unsolicited valid non-serious case received from a other health professional. This case involves a 54 years old female patient who developed fever of 103.9? fahrenheit, weakness and Influenza quadrival A-B high dose HV vaccine [Fluzone high-dose quadrivalent] was inadvertently given to a patient who was under the age of 65. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included: Bivalent COVID booster for Immunization. On 09-Nov-2022, the suspect Influenza quadrival A-B high dose HV vaccine at a dose of 0.7 ml (lot UJ926AC, expiration date: 30-Jun-2023) via intramuscular route in the left deltoid for Immunization was inadvertently given to a patient who was under the age of 65 (product administered to patient of inappropriate age) (latency: same day). On 09-Nov-2022 the patient developed fever of 103.9? fahrenheit (pyrexia) and weakness (asthenia) on the same day following the administration of Influenza quadrival A-B high dose HV vaccine. Relevant laboratory test results included: Pyrexia - On 09-Nov-2022: 109 ? Fahrenheit. Action taken: Not applicable. Corrective treatment: Not reported. Outcome: Unknown for all the events. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the guidelines. Patient received 2 flu shots in the same season. Patient came in the pharmacy on 11/23/2022 asking for the flu shot. He received the flu shot that day. On 11/25/22, the pharmacist reviewed his medication list and realized patient already received the flu shot on 08/24/2022. Then pharmacist met patient again on 11/25/2022 to provide COVID booster shot. On that day, patient reported having no unusual symptom. Patient was in good spirit.
