Patient received vaccines at approximately 09:30, and waited in the common area under observation until roughly 10:00. Patient then asked to go to his room, which he was granted, as his waiting period was over. Patient was then found dead at approximately 12:00pm by Facility Staff when they went to get him for lunch. Reason for Death is unknown at this time.

Symptomtext

Patient received his influenza vaccine on Wednesday October 19th. On Thursday October 20th, he reportedly did not feel well and developed a cough, shortness of breath, and a fever. He was seen at a local urgent care where he received a breathing treatment and a prescription for Albuterol HFA and Capron DM. The provider told the patient to "take it easy". On Monday October 24th, the patient called his primary care physician's office, with the primary complaint of shortness of breath. A nurse instructed patient to get checked out at the ER. Patient reported his oxygenation level was 85 at the ER and was the reasoning for his admission to the hospital. His fever was reportedly 102.8F. He underwent multiple tests and received a steroid shot before being released the following day, Tuesday October 25th. He was discharged with a prescription for Azithromycin 500mg - Take one tablet by mouth once daily (2 day supply). Patient is currently at home monitoring his oxygenation with a finger pulse oximeter. He reports his oxygenation level is around 90. His follow-up appointment with Dr. is on November 3rd.

11/9/2022 ~ 9:30 p.m. Minor headache, maybe low fever, treated with 500 mg Acetaminophen, stuffy nose - no treatment. 11/10/2022, 1:35 a.m. sneezing and runny nose, mild, not treated, and headache/mild fever treated with another 500 mg Acetaminophen at 2:50 a.m. At 3:10 a.m. ~2.5 mg zolpidem for difficulty sleeping. 11/10/2022 cont. 09:45 continued mild sneezing, runny nose a bit worse, no treatment done. 500 mg Acetaminophen at 1:35 p.m. Headache less, but mild malaise, feeling sick, from about 2:00 p.m. Am not up for much activity.

redness at the injection site and surrounding area; Initial information received on 24-Oct-2022 regarding an unsolicited valid non-serious case received from a pharmacist. This case involves a 71 years old female patient who had redness at the injection site and surrounding area after receiving vaccine INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE [FLUZONE HIGH-DOSE QUADRIVALENT]. The patient's past medical history, medical treatment(s)and family history were not provided. Patient had no concomitant and other vaccines. On 22-Oct-2022, the patient received a 0.7ml 1X(once) dose of suspect INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE (strength, formulation was unknown, lot UJ932AD and expiry date: 30-Jun-2023) via intramuscular route in the right arm for prophylactic vaccination. On 24-OCT-2022 the patient developed a non-serious event of redness at the injection site and surrounding area (vaccination site erythema) 2 days following the administration of INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE. Patient had no swelling, no itchiness, no change in breathing, only redness. Action taken: not applicable. It was not reported if the patient received a corrective treatment for the event Outcome: Not Recovered

2 doses of FLUZONE HIGH DOSE QUADRIVALENT to a patient 20 minutes apart with no reported adverse event; Initial information received on 15-Dec-2022 regarding an unsolicited valid non-serious case received from a consumer/non-hcp. This case involves a 71 years old female patient who took 2 doses of INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE [FLUZONE HIGH-DOSE QUADRIVALENT] 20 minutes apart with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 15-Dec-2022, the patient received 1st 0.7 ml dose of suspect INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE (lot: UJ932AD) via intramuscular route in the left deltoid and received 2nd 0.7 ml dose of the same vaccine via intramuscular route in the right deltoid for Immunization. On an unknown date the patient administered 2 doses of fluzone high dose quadrivalent to a patient 20 minutes apart with no reported adverse event (inappropriate schedule of product administration) (unknown latency). Action taken: not applicable. It was not reported if the patient received a corrective treatment for the event. At time of reporting, the outcome was Unknown for the event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices.
