Symptomtext

Developed severe generalized pain in chest, most severe centrally around 10 pm. Pain scale 8/10. No nausea or sweating BP 200/98. Rechecked in approximately 10 minutes and had decreased to 160/82. Pain lasted about 30 minutes, lessening gradually. Felt extremely weak afterward. Continuing to have weakness and lethargy today (pain last night), but no pain.

5/15/23 I started with a cough that became more persistent. No other symptoms. The next morning, I had congestion, headache, body ache, continued coughing, felt really sick and felt like I had been hit by a truck. I was taking many over the counter medications. On 5/17/23 I took an at home COVID test and it was positive. I began taking Paxlovid and began feeling much better. I still have lingering allergy like symptoms.

The patient says she feels completely fine no side effects at all and still wants her covid booster. She was advised to come 2 weeks from 12/15/22

Severe whole body hives; This is a spontaneous report received from a contactable reporter(s) (Other HCP). The reporter is the patient. A 67-year-old male patient received BNT162b2, BNT162b2 omi ba.4-5 (BNT162B2, BNT162B2 OMI BA.4-5), on 11Nov2022 at 08:45 as dose 1, single (Lot number: Gj6738) at the age of 67 years, in right arm for covid-19 immunisation; influenza vaccine inact split 3v (FLUZONE HIGH DOSE), on 11Nov2022 as dose 1, single (Lot number: Uj934aa), in left arm for immunisation. The patient had no relevant medical history. The patient's concomitant medications were not reported. The patient has no known allergies and no covid prior vaccination or other vaccine in four weeks. The following information was reported: URTICARIA (medically significant) with onset 13Nov2022 at 20:00, outcome "not recovered", described as "Severe whole body hives". The event "severe whole body hives" required physician office visit. Therapeutic measures were taken as a result of urticaria including Prednisone 40 mg per day. The patient was not tested for covid-19 post vaccination.; Sender's Comments: As per the available information the causality of the event urticaria with the suspect BNT162b2, BNT162b2 omi ba.4-5 can not be ruled out completely. The impact of this report on the benefit/risk profile of the Pfizer product is evaluated as part of Pfizer procedures for safety evaluation, including the review and analysis of aggregate data for adverse events. Any safety concern identified as part of this review, as well as any appropriate action in response, will be promptly notified, Ethics committees, and Investigators, as appropriate.

Patient came into the vaccine clinic asking for a flu shot. We asked him about the last time he got his flu shot and he said that his most recent flu shot was last year. We also asked his family members about his last flu shot and they agreed that his last flu shot was indeed last year. We took their word without checking his vaccine records and gave him the flu shot. Patient left the vaccine clinic afterwards without any symptoms.

vaccine ended up on the patient's neck and shirt with no reported adverse event; did not receive the entire dose with no reported adverse event; Initial information received on 14-Nov-2022 regarding an unsolicited valid non-serious case received from a physician. This case involves a 73 years old male patient who did not receive the entire dose and vaccine ended up on the patient's neck and shirt with no reported adverse event while receiving vaccine INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE [FLUZONE HIGH-DOSE QUADRIVALENT]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included COVID-19 VACCINE (COVID-19 VACCINE) for Prophylactic vaccination. On 11-Nov-2022, the patient received 0.7 mL dose of suspect INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE (lot UJ934AA and expiry date: 30-Jun-2023) via intramuscular route in the right deltoid for prophylactic vaccination. On 11-Nov-2022, (latency: same day) the patient did not receive the entire dose with no reported adverse event (incorrect dose administered) and vaccine ended up on the patient's neck and shirt with no reported adverse event (accidental exposure to product) following the administration of INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE. It was reported, when administering FLUZONE HD QIV to a patient. When she administered the vaccine, there was almost no resistance when pushing on the plunger. A lot of the vaccine ended up on the patient's neck and shirt. The patient noted something cold on his neck and the pharmacist noticed a visible wet stain on the collar of the patient's shirt. Action taken with QUADRIVALENT INFLUENZA VACCINE (FLUZONE HIGH-DOSE QUADRIVALENT) was not applicable. At time of reporting, the outcome was Unknown for both the events. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices.
