The patient completed a paper flu consent form and answered "yes" to the question of whether she was pregnant or could become pregnant in the next month. She reported that her menstrual cycle was 7 days late, so she requested the preservative-free flu vaccine out of precaution. The RN nurse who screened her consent noted the request for preservative-free flu vaccine. The RN vaccinator mistakenly administered Fluzone Quad instead of preservative-free Fluarix. This was a medication error; no adverse events have occurred per our knowledge. Another RN counseled patient post-medication error.
