Symptomtext

patient came in for a well child visit on 12/21/2023. Was supposed to receive quadracel and Proquad. Patient received quadracel and pentacel instead. Called and reported error to the state and was advised patient may experience an exaggerated reaction (Increased tenderness, swelling, redness), adverse event brought to the attention of the covering provider. Per the state and Dr. parent was advised no harm in receiving extra dose of vaccine, just may see an increased risk for exaggerated reaction (above reaction discussed in detail above) Parents verbalized good understanding and will call with any additional questions or concerns. Patient is scheduled to come back for the proquad in December.
