Symptomtext

Scheduled for a #2 Hepatitis B vaccine, but no record of Hepatitis B on record. Spoke with patient, she stated she had Hep B series as a child and just needs 2nd Hepatitis A. Hepatitis A vaccine given to patient. Printed record and it showed she needed another Hepatitis A vaccine. Spoke with another nurse and investigated further to realize the dose had been given 2 months earlier than recommended. Called patient to notify her of error and explained she would need to come back after April 28, 2023 for repeat Hepatitis A vaccine. Patient verbalized understanding. Charted this information in chart.
