Symptomtext

Patient reports consistent pain in site of injection, as well as inability to sleep on left side. Reports subjective limited range of motion with left arm and pain with other various shoulder movements. Patient (who is a healthcare professional) is concerned that vaccine was administered too high into the bursa and not deltoid muscle.

Received at 1:30pm 1:35pm became reactive. 5 mins after injection became itchy all over body, Itchy throat, Rash on face, Wheezing, Chest tightness, Shortness of breath 1:45pm Took 1 Benadryl. Spoke to Dr. she sent me straight to the ER and was admitted to the ED at 1:50pm. Was given another Benadryl with 2 steroids pills, and Pepcid, and a breathing treatment. Stayed in ER for 2 hours then sent home with refill on my inhaler and Methylprednisolone pack to start on 11/23/21 if symptoms did not improve, and told to continue Benadryl every 4-6 hours as need. I still had symptoms started Methylprednisolone pack on 11/23/21.

Symptomtext

Patient had just received flu vaccine to left deltoid and within minutes (<5 min) had redness/rash, itching and swelling near injection site. She felt her heart racing and some dizziness. Denied shortness of breath of chest discomfort initially. The rash/redness/itching started to travel up to her chest/neck area and down her left arm. She was wheelchaired to the ER for immediate intervention. Upon arrival to ER triage area, she did report some tightness in her chest and started to feel some throat discomfort, but denied shortness of breath or difficulty breathing. In ER: Temp 98 degrees, HR 82, RR 18, pulse ox 99% on room air, BP 139/82 She was given Decadron 10mg IV push, Pepcid 20mg IV push & Benadryl 50mg IV push. Upon re-evaluation, chest tightness, chest wall pain & redness to left arm was resolving. On discharge from ER, she was given Rx for Prednisone x 7days and instructed to follow-up with PCP.

Child with sibling at well visit and should have received flu vaccine only. Child was mistakenly given the DTAP/IPV (Quadracel) during the visit (was intended for 4 y/o sibling) Nurse administered Quadracel first and mom went to lift child off table and nurse still had flu vaccine to give, which was administered.
