Patient received Janssen immunization on 11/26/2021 at 6:20 pm. About a minute or so after receiving the shot, the patient had an adverse reaction to the vaccine. Gave the vaccine, walked out of the immunization booth to print a scan tag, then returned to the booth with the scan tag. Upon entering immunization booth patient appeared slumped in chair with head back and eyes open, but unfocused. Patient was moving erratically around in chair. Called out to patient and patient did not respond. Came up to patient, touched shoulder, and patient still did not respond. Patient screamed out and seemed to be in a panicked state. Touched shoulder again and said patient's name and patient turned to look and her eyes became more focused. Patient seemed to slowly calm down after that. Patient claimed to be dizzy and lightheaded after that. Patient described the event that she could feel the vaccine moving through her body. Patient stated that she felt like she was in her head screaming and then she was seen. Patient seemed confused about what happened after the incident and breathing was labored/erratic. Patient cried and comforted patient for about 10-15 minutes after incident.

Pt received annual flu shot and then sat down on blood pressure machine to take BMI. As she was sitting there she got lightheaded, hot and nauseous. Pt proceeded to pass out and bump her head on the machine and slide off of the seat onto the floor. Gave pt a chair to sit in and orange juice. She sat there until her color came back and all other symptoms disappeared.

Vaccinator pushing the needle into the arm she was pushing on the plunger at the same time, so part of the medication went on his arm and the subcutaneous tissue into the muscle. with no adverse event, the full dose was not administered; Initial information received on 02-Nov-2021 regarding an unsolicited valid non-serious case received from a pharmacist via Regulatory Authority (under the reference 00838646). This case involves a 47 years old male patient whom vaccinator pushing the needle into the arm, pushing on the plunger at the same time so part of the medication went on his arm and the subcutaneous tissue into the muscle (exposure via skin contact) and the full dose was not administered (underdose) while receiving INFLUENZA QUADRIVAL A-B VACCINE [FLUZONE QUADRIVALENT]. The patient's medical history, medical treatment(s), vaccination(s) and family history were not provided. Patient had no other concomitant vaccines. On 01-Nov-2021, the patient received a first dose of suspect FLUZONE QUADRIVALENT (total, once, lot UT7377LA and expiration date: 30-Jun-2022) via intramuscular route in the left deltoid and 2nd dose were administered on unknown dates (lot number and expiration date not provided) via intramuscular route in right deltoid for prophylactic vaccination. It was an actual medication error due to exposure via skin contact and underside (same day latency). No adverse event was reported. It was reported "Caller stated that they had an administration error involving FLUZONE QUAD PFS. the full dose was not administered to the adult patient. the vaccinator plunged the plunger before injecting the vaccine. she repeated the dose, but needs guidance for future reference. Caller states yesterday she was injecting a consumer and as she was pushing the needle into the arm she was pushing on the plunger at the same time so part of the medication went on his arm and the subcutaneous tissue into the muscle. Caller repeated the dose yesterday and called the Communicable Disease Center (CDC) to find out what to do." At time of reporting, the outcome was Unknown for the reported events. There would be no information on the batch number for the another dose.

I am asymptomatic (and have been the duration of my positive diagnosis), but was in close contact with my boyfriend who was positive for COVID 19 and was showing symptoms.

A couple came in to receive Pfizer COVID boosters and the wife requested a flu shot at the same time. I got both patients ready and scanned their forms into the handheld, and scanned the correct items (flu shot box QR code and manual input for the pfizer shot). When all info was input into the system, I went to give the shot to the husband first, and misakenly grabbed the prefilled flu syringe instead of one of the pfizers I had drawn up. I had an intuition I made the mistake and asked the couple who wanted the flu shot to confirm. They also noticed that I gave the odd vaccine to the husband instead of the wife who requested it. The husband states that on 10/25 at pharmacy he received this year's flu shot. I confirmed it was this year's afluria. He was not concerned and stated getting a double vaccine should not hurt him. I confirmed this and said he should be fine, but asked them to stay around for 15 minutes to ensure there was no side effect and to get any extra info from them while I filled out the error reporting forms.
