Symptomtext

Patient is in the age group to have had the COVID Vaccine for 5-11 years old. Instead, the patient received the wrong vaccine, being given instead the COVID vaccine for ages 12 and older. There were no symtoms at the time adminsitration. Healthcare staff is trying to reach patient to notify parents of the error and check on patient. Error was identified when the nurse started to document in system and recognized that the age and the vaccine lot number corresponded to the other age group. The professional that administered the wrong vaccine is workign with the clinic to map out process cahllenges and make improvments to prevent another error from occurring. The patient and family will be offered the opportunity to have a follow-up appointment with a provider to discuss concerns or answer more questions. It will be recommended, per guidelines, for the patient to recieve the second dose in 21 days.
