On Monday pt received Quadracel and Flu in L leg, Proquad in R. Mom called today stating she believes it patient's L leg. It is swollen and the diameter of an orange, is very warm, hard and a little firm at the site. Tuesday morning it was size of a nickle, by bedtime the diameter was the size of a golf ball and woke up with a diameter the size of an orange today, 9/21/22. Only other red marks are on a foot and on the back. Mom thinks those are mosquito bites. Pt is not having any difficulty breathing. No swallowing issues. Originally scheduled for an in office at 2 pm with , FNP . Because of transportation issues, switched to telemedicine. Mom has been using cold compresses on site and will continue to do so. Per the visit: localized superficial swelling of skin (Left) Child developed localized reaction s/p Dtap, IPV, and flu vaccine in L thigh 24 hours after vaccines given as evidenced by mild to moderate erythema currently 4 inches in diameter without tenderness or decreased ROM of extremity. Child did not have respiratory distress, swelling of face, wheezing or hives, so localized and minor. Discussed with mother over video visit that reaction is likely related to Dtap vaccine but flu vaccine is also possible culprit and is not indicative of allergy to vaccine components. Since erythema and warmth at the injection site is improving, advised to continue to use cool compresses at the area and monitor for danger signs as discussed. Mom to call hospital if noted.
