Symptomtext

Patient received Flu and COVID vaccines in clinic today. Patient waited the appropriate time after immunizations. As the patient was checking out it appears she had passed out. By the time this nurse got to the other side of the check out desk, patient was alert and responding. The nurse took the patient back to the exam room and gave juice and crackers. Patient waited an additional time and left with parents once they felt comfortable.

Symptomtext

Pt. received Flouzone (purchased vaccine) at 1:45 pm and reports that she started having diarrhea that started about 2:20 pm after receiving the Flu vaccine. She then developed sore throat and pain with breathing shortly about 2:30 pm. The next day she had a bump on bottom lip that was hard to the touch that came up and was about 5 mm in size. She had diarrhea that lasted 6 days, sore throat (swollen and tight feeling) that lasted 3 days. She took ibuprofen, chroraceptic and kaopectate. She is now recovered.

"Acute onset of illness at about 0130 with crying and fussiness, poorly consolable and vomited a short time later. Patient seemed to have abdominal pain. Ultrasound obtained which demonstrated INTUSSUSCEPTION. Total of 2 episodes of vomiting. No stools. No fever or other signs or symptoms of illness. Patient was his usual self yesterday. Otherwise generally healthy." "Ultrasound here demonstrates persistence of the INTUSSUSCEPTION - reduced successfully by contrast enema. Able to take and retain po fluids without problems. Will discharge with care instructions."

Symptomtext

Patient called today and said she was administered a flu vaccine on Wednesday, Sept 28th at a flu clinic our store was part of. She called today and said that her arm was swollen at the injection site and was red, slightly warm to touch. Also there was a rash around the area the size of a softball No breathing issues. Patient saw a nurse and school and the nurse suggested she contact us. Told her to put ice on it and take benadryl. If still an issue on Monday or if it got worse- see a doctor.

Immunization counseling provided. Discussed TdaP, MMRV, Hep. B, and IPV as well as influenza. Components discussed. Discussed risks and benefits. After immunization administration, printed updated immunization record from registry. At that time, discovered that child had received other immunizations that were not recorded in our records. Thus, received vaccines that were not due or necessary (TdaP, MMRV, Hep. B, and IPV). Informed father and apologized; discussed that he had received vaccines since the time we saw him last and dad was unaware of that. Also, the Hep. B received in June 2016 was not recorded in the registry printout from December 2020 that we used to enter vaccines into our records. Vaccines ordered at time of visit were based on information in records at the time. No adverse outcomes expected from getting extra protection through vaccines administered other than usual temporary risks of vaccines such as soreness.

Symptomtext

Patient under 6 months of age was administered Fluzone QIV with no reported adverse event; Initial information received on 20-Oct-2022 regarding an unsolicited valid non-serious case received from a consumer/non-hcp. This case involves a 12 weeks (3 months and 5 days) old female patient who was under 6 months of age and administered INFLUENZA QUADRIVAL A-B VACCINE [FLUZONE QUADRIVALENT] with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 20-Oct-2022, the patient received INFLUENZA QUADRIVAL A-B VACCINE Suspension for injection (Frequency: once) (strength: standard) (dose, expiry date was unknown) (lot no. UT7689KA) via intramuscular route in the right thigh for Immunization. On 20-OCT-2022 the patient under 6 months of age was administered fluzone qiv with no reported adverse event (Product administered to patient of inappropriate age) (latency: same day). Action taken: not applicable. At time of reporting, the outcome was Unknown for the event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
