Inappropriate dose of vaccine administered/ patient was accidentally administered a 5th dose of Prevnar 13; wheezing associated respiratory infection; upper respiratory tract infection.; This is a spontaneous report received from contactable reporter(s) (Nurse), Program ID. A 16-month-old male patient received pneumococcal 13-val conj vac (dipht CRM197 protein) (PREVNAR 13), on 28Oct2022 as dose 5, 0.5 ml single (Lot number: FL4125, Expiration Date: 30Jun2024) at the age of 16 months intramuscular for immunisation; influenza vaccine (FLUZONE [INFLUENZA VACCINE]), as dose 1, single (Batch/Lot number: unknown), as dose 2, single (Batch/Lot number: unknown), on 26Sep2022 as dose 3, single (Batch/Lot number: unknown) and on 28Oct2022 as dose 4, single (Lot number: UT7701NA, Expiration Date: 30Jun2023) for immunisation; diphtheria vaccine toxoid, hib vaccine conj (tet tox), pertussis vaccine acellular 5-component, polio vaccine inact 3v (mrc 5), tetanus vaccine toxoid (PENTACEL), as dose 1, single (Batch/Lot number: unknown), as dose 2, single (Batch/Lot number: unknown), as dose 3, single (Batch/Lot number: unknown), as dose 4, single (Batch/Lot number: unknown) and as dose 5, single (Lot number: UJ681AA, Expiration Date: 09Sep2023) for immunisation; hepatitis a vaccine inact (VAQTA), on 21Jun2022 as dose 1, single (Lot number: W021491, Expiration Date: 14Dec2023) and as dose 2, single (Batch/Lot number: unknown) for immunisation. The patient's relevant medical history included: "diagnosed allergies" (unspecified if ongoing); "respiratory illness" (unspecified if ongoing); "genetic/chromosomal abnormalities" (unspecified if ongoing); "endocrine abnormalities" (unspecified if ongoing); "obesity" (unspecified if ongoing); "compromised immune status" (unspecified if ongoing); "diabetes" (unspecified if ongoing). The patient's concomitant medications were not reported. Vaccination history included: prevnar 13 (Dose 1, Single, Lot: EE7117, Expiry: 31May2023, NDC: 0005-1971-01, 0.5mL intramuscular injection to right vastus lateralis), administration date: 16Aug2021, for Immunization; prevnar 13 (Dose 2, Single, Lot: EE3120, Expiry: 31May2023, NDC: 0005-1971-01, 0.5mL intramuscular injection to left vastus lateralis), administration date: 14Oct2021, for Immunization; prevnar 13 (Dose 3, Single, Lot: EJ4560, Expiry: 31Jul2023, NDC: 0005-1971-01, 0.5mL intramuscular injection to left vastus lateralis), administration date: 14Dec2021, for Immunization; prevnar 13 (Dose 4, Single, Lot: FF7356, Expiry: 30Apr2024, NDC: 0005-1971-01, 0.5mL intramuscular injection to left vastus lateralis), administration date: 26Sep2022, for Immunization. The following information was reported: INCORRECT DOSE ADMINISTERED (non-serious) with onset 28Oct2022, outcome "unknown", described as "Inappropriate dose of vaccine administered/ patient was accidentally administered a 5th dose of Prevnar 13"; UPPER RESPIRATORY TRACT INFECTION (non-serious) with onset 28Oct2022, outcome "unknown", described as "upper respiratory tract infection."; WHEEZING (non-serious) with onset 28Oct2022, outcome "unknown", described as "wheezing associated respiratory infection". Additional information: the patient was accidentally administered a 5th dose of Prevnar 13. No other conditions. This child patient who received many doses of Prevnar 13. This child received a total of 5 doses of Prevnar 13 vaccines. She clarified that this 16 month old patient was accidentally administered a 5th dose of Prevnar 13. There was another male child around this patient's age at the facility at the time who was supposed to receive the vaccine given to this patient; that additional male child did not miss their doses, they received the intended vaccines as prescribed. She called for advisement for this child. Sick visit, diagnosis given was wheezing associated respiratory infection and upper respiratory tract infection.
