Symptomtext

Presented to the ED with syncope and found to have bradycardia with sick sinus syndrome with recurrent pauses >5 seconds while awake. Permanent pacemaker placed # Sinus pauses # Troponin elevation # Incomplete RBBB - resolved 7s pause during sleep then 17s pause during blood draw. Syncope with 17s pause, emesis afterwards. Second event was possibly related to an exaggerated vagal response. Incomplete RBBB noted on second EKG 9/14 AM, sinus brady on repeat after recent pause 9/15 AM. Troponin elevated on admission but stabilized, repeat after sinus pause remains stable. Not nodal blockers at home. No recent illness, chest pain or typical EKG changes or pericardial effusion on prelim echo to suggest myopericarditis. No history of infiltrative disease, normal real function.

Symptomtext

Patient awoke around 11:00am on 11/20/22 to find a painful rash mostly on the palmar sides of both hands. There is no rash at any other location. The rash is described as small, interrupted round, raised, red spots of multiple sizes and painful, no open wounds or blisters. Treatment prescribed is triamcinolone acetonide topical cream 0.5%, Prednisone 20mg two tablets orally once daily for 3 days and mupirocin 2% ointment.

PT CALLED AND STATED THAT SHE GOT REALLY DIZZY AND LIGHTHEADED AFTER THE VACCINATION. SHE STATED THAT SHE WAS SO DIZZY THAT SHE FELT LIKE SHE WAS DRUNK. SHE HAD HER BOYFRIEND DRIVE HER TO HER DOCTORS OFFICE. SHE STATED THAT IT MIGHT HAVE BEEN DUE TO THE PREDNISONE THAT SHE HAD TAKEN. SHE WAS DISTRESSED WHEN RECIEVING THE VACCINE AND MAY HAVE BEEN EXPERIENCING EXTREEME ANXIETY AS WELL.

Symptomtext

After patient left office arm were the vaccines were given started to turn red with irritation. Next day patient experienced headaches, chills, ear aches, joint pain, and loss of appetite. Rash grew to cover most of the upper arm with swelling.

fluzone high dose qiv was inadvertently administered to a 6 month old infant with no reported adverse event; Initial information received on 13-Oct-2022 regarding an unsolicited valid non-serious case received via health professional. Agency number: 01321030 This case involves a 6-month-old female patient to whom vaccine Influenza Quadrival A-B High Dose HV Vaccine [Fluzone High-Dose Quadrivalent] was inadvertently administered with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included Diphtheria Vaccine Toxoid, HIB vaccine CONJ (TET TOX), Pertussis Vaccine Acellular 5-Component, Polio Vaccine INACT 3V (MRC 5), Tetanus Vaccine Toxoid (Pentacel) for Prophylactic vaccination; ROTAVIRUS VACCINE for Prophylactic vaccination; and Pneumococcal Vaccine 13V for Prophylactic vaccination. On 13-Oct-2022, the 6-month female patient inadvertently administered Fluzone High-Dose Quadrivalent vaccine at dose 0.7 ml once via intramuscular route in the right thigh (lot: UT7718CA, expiry date: 30-Jun-2022) for immunization with no reported adverse event (product administered to patient of inappropriate age) at same day latency. The reported inquired what they should look out for; asks if it will cause harm to the infant? Action taken: Not applicable. No corrective treatment was received for the event. At time of reporting, the outcome was Unknown. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
