Employee reported to COVID-19 Bivalent and Influenza Clinic. Employee completed influenza vaccination record form and the nurse administered. A pre-filled COVID-19 Bivalent consent form had been completed by employee and was noted to be incomplete near end of clinic. Nurse called employee and it was discovered that she received influenza vaccine instead of COVID-19 Bivalent vaccine. Employee reported she received flu vaccine at provider's office the week prior.
