Symptomtext

Mother called about child. Said his left arm, Deltoid, was red and swollen. I explained child probably tighten up his arm, muscle when shot was given plus that arm got Dtap along with Hep A. She also said his right arm hurt, Deltoid where he got his flu shot along with a Proquad-SQ. She also said where the flu shot was given, it was a little itchy. Told mother to put cool compress on it give with Tylenol or Motrin prn. If the arm is still itchy, give Benadryl prn. Any other problems call PCP

Dizziness Eye movement disorder

Symptomtext

Nurse entered room to vaccinate patient with COVID Pfizer Bivalent Booster, Menquadfi, and Fluzone. Patient stated that last year after receiving her flu vaccine she passed out. Nurse asked patient if she had anything to eat/drink prior to visit and patient stated she had. Nurse gave patient flu and menquadfi in left arm. Patient was feeling ok and nurse then gave COVID into the right arm, as needle was being capped patient stated "I am feeling woozy". Patient's eyes rolled in back of head, nurse laid bed down and raised her legs. Patient was given apple juice and a snack. Patient left office 20 minutes after event occured.

Symptomtext

Patient was given a flu vaccine and was being monitored for 30 minutes due to an allergy to eggs. The administration was approved by a doctor. 20 minutes in the patient started to break out into a rash and the nurse retrieved a doctor to come check on the patient. The patient was given Zyrtec and was monitored for another 30 minutes. After the timer went off the patient was sent home with instructions to parents to monitor.

Myalgia Pain Pain in extremity

Symptomtext

Client experienced excruciating pain in left arm within a few hours of administration. Describes pain as muscular. Client has full range of motion of joint. No swelling, redness, or warmth noted on 11/7/2022. Client continues to experience extreme pain especially with movement of extremity. Lidocaine patches provide some relief. Tylenol and Motrin provide little relief. Encouraged client on 11/07/2022 to follow up with primary care provider for additional evaluation and treatment.

Symptomtext

a 13 month old patient inadvertently received 2 doses of FLUZONE QIV NP 13 days apart with no reported AE; Initial information received on 21-Feb-2023 regarding an unsolicited valid non-serious case received from a physician. This case involves a 13 months old and unknown gender patient who had inadvertently received 2 doses of influenza quadrival A-B vaccine [Fluzone QIV] 13 days apart with no reported ae. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On an unknown date, the patient received an unknown dose of suspect influenza quadrival A-B vaccine, suspension for injection (strength unknown) (lot: UT7723LA; expiry date: 30-Jun-2023) via unknown route in unknown administration site for Immunization. On an unknown date the patient had received 2 doses of influenza quadrival A-B vaccine 13 days apart with no reported ae (inappropriate schedule of product administration) (same day latency). Action taken with quadrivalent influenza vaccine (Fluzone QIV) was not applicable. At time of reporting, the outcome was Unknown for the event a 13 month old patient inadvertently received 2 doses of fluzone qiv np 13 days apart with no reported ae. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the guidelines. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

Symptomtext

The vaccine listed above (bivalent) vaccine was inadvertently administered as first COVID vaccine dose for a five year old patient as opposed to the monovalent vaccine. The patient would have received this dose as a third dose-booster. Provider recognized shortly after while the patient was still in the office with parent/guardian. Parent/guardian informed of situation and follow up. Wrong dose inadvertently administered, no adverse event.

Flu vaccine given to patient by accident instead of the covid vaccine (this was patient's second flu vaccine of the season). Spoke with Dr. about it before giving the covid vaccine, and Dr. gave ok to give the covid vaccine along with the flu vaccine

Symptomtext

Approximately 2 minutes after vaccine was given and while in observation. Patient was observed sitting in a chair and falling over. Patient did not lose consciousness and remained on the floor for immediate care. Two providers came to assist including the clinic manager. Normal breathing and respiratory were noted. Patient remained on the floor with knees elevate, later lowering the knees to a bending position. Position was then corrected to a sitting position in a chair. Patient was moved to the school nurse's office where she was given juice and a location to lay down and be observed for 30 minutes. School nurse notified parent/guardian.
