Symptomtext

Brief seizure occurred shortly after vaccines were administered as patient was leaving the office. He recovered quickly and had no lingering effects. His Depakote level was checked later in the day and found to be low.

Symptomtext

L. deltoid influenza vaccine administered to employee on 3/23/23. no bleeding noted after administration. She denies redness, swelling, or bruising post administration. She reports "I started having pain about two days after I received the flu vaccine to my L arm right below where the needle was inserted. It is tender when I push on it and it's specifically more sore in the morning when I wake up. I especially notice it when I try to take off a shirt I can't hardly raise my arm up I have to use my Right arm to take my shirt off. I rate my pain 5/10 in the morning and 2/10 throughout the day." Issue lasted 2.5 months and has since resolved.

Symptomtext

A patient received the pfizer bivalent booster and fluzone quad vaccine. She came back to the pharmacy about 20 minutes after receiving the vaccines and reported she had some trouble breathing and she used her inhaler. She reported she felt somewhat better after using her inhaler. Since she was not feeling completely better she decided to go to urgent care to get evaluated.

Symptomtext

Red scaly rash developed on December 31, 2022 on chest, then spread to arms, stomach, and back in the days and weeks following. On January 17th, 2023 I saw my doctor, and it was then diagnosed as pityriasis rosea. As of February 15, 2023 the rash is still present, but getting better. Some scarring is occurring with the rash spots, but the color is fading to skin tone on the first of the spots. The rash spots that did not develop initially are still red.

Visual exam on January 17, 2023 by my doctor and another doctor in the office diagnosed the rash as pityriasis rosea. Because herpes is a possible cause of the rash, I took a blood test for herpes (HSV 1 and 2 IGG) on January 19, 2023. That test came back negative with no previous exposure to herpes.

Symptomtext

dizziness, nausea, sweating within minutes of vaccine administration - patient remained seated, visibly sweating, drank some water, started feeling better within 5 minutes. Remained in store 15-20 minutes and was feeling back to normal when left with family. Would contact Dr. if returned or continued.

Symptomtext

Moderna Monovalent 0.25 mL was administered as booster dose instead of Moderna Bivalent 0.5 mL. Pt. tolerated well, there was no adverse reaction at time of vaccination and after. Once error was caught by RN Manager - pt. was contacted and made aware of the above.
