Symptomtext

Approximately 2 minutes after receiving vaccines, at 10:35am, while sitting in parent's lap, pt had what appeared to be a tonic seizure x 10 seconds. Pt did not sustain any injuries before/during/after seizure. Immediately following seizure, pt was able to state his name, squeeze his mother's hand when prompted, and nod in agreement to questions. Pt was alert and oriented but fatigued, stated he didn't want to sit up because he was tired. Per parent, pt did eat breakfast that morning. Pt appeared pale and clammy. Manual BP was taken (82/62 right arm, sitting) and temp (96.3 oral), slightly decreased. Snacks and juices given, which pt consumed readily. Pt complained of nausea for a few minutes and then resolved. Parent was given information for ED and nearby urgent cares, and offered to call EMS. Parent declined. After about 40 minutes in observation pt was standing and walking around, color returned, speaking regularly. BP taken again, improved (96/70). Parent called pediatrician and set up appt for 4pm same day. Pt ambulated out of clinic with parent, alert, oriented, and steady on feet. Per parent, pt had appt with pediatrician same day as event at 4pm, physical exam was within normal limits. No testing was done. Pt has appt scheduled with neurology in 2 weeks.

Systemic: Fainting / Unresponsive-Severe, Systemic: Seizure-Severe, Additional Details: After vaccination, mother said she is being evaluated for epilepsy and this was the 3rd or 4th time patient has had a seizure after receiving a vaccine. She first received the Pfizer covid booster, and then moments later received the flu shot. She showed no symptoms of seizure/fainting after receiving the covid booster, but after receiving the flu shot, she briefly lost consciousness and began shaking for approximately 10-15 seconds before regaining consciousness. Dr. office notified.

11/14/2022 @ 1600 Extreme fatigue, difficulty driving home, napped for 1 hour and had to sleep more than 9+ hours on this day. Usually sleeps for 7.5 hours. Subsequently slept for 9 + hours. 11/15/2022 Persistent daily headaches of moderate intensity x 12 days. 11/16/2022 Onset brain fog, much harder to concentrate and focus 11/17/2022 Onset symptoms of a stuffed-up nose began with minor nose bleeds. No rhinorrhea is persistent and continues. Arthritic pain in wrist, hips, and knees. Moderate pain and generalized muscle weakness began. 11/18/2022 Onset dry cough, current and persistent at this time, chills and fever with 99.8 when the normal for the patient is 98.2, accompanied by early morning sweats and lasting another 7 days. Also, brain fog intensified on this date and lasted for another x7 days. 11/20/2022 Onset of skin rash on forelegs with urticaria and skin over body became very sensitive and painful to touch. The pain side effect persists, but the rash and urticarial have abated?Self-medicated with Atarax 10 mg 1 tab PO. 11/24/2022 Vertigo and labyrinthitis symptoms appeared but gradually abated over 48 hours. 11/25/2022 Temperature remained normal all day. Brain fog began to improve. Persistent symptoms: The Dry cough continues at present. Bilateral hand pain and mild swelling persist. The painful and sensitive skin is still tender, but the rash is gone?no more urticaria. General muscle strength is still weaker, and the bilateral hand grip is only 60 % of normal.

Patients were mentally delayed brothers with similar names and they told me the wrong name. The patient's mother stated that they do this all the time and she apologized for not catching it. Patient received an HPV vaccine that his brother should have received. They both also received tetanus and flu vaccines.
