9/22/2023 ''I had a flu shot 2 weeks ago, and it was the most painful shot I ever had. It was painful when she put the needle in and then when she pushed the plunger it was very painful all the way from my shoulder to my hand.'' Patient states reports the pain as 4/10 constant, more so at night. In addition, ''If I touch at the joint at the top of my shoulder it is painful 4/10 tender.'' States no redness, no swelling, no swollen glands in the axilla. Reports he is able to move the shoulder normally, but the pain is there and worsens after use during the day. I have tried to call him to f/u but cannot reach him. Will try again.

- 1. Screening status Colorectal CA: 9/3/09 CSP w/ 1 hyperplastic polyp. 12/21/18 CSP w/lymphoid aggregate polyp. Repeat 2028. Prostate CA: 0.27 PSA 2011. 0.466 PSA 2018. 0.51 PSA 2023. Hep C: 2019 negative. AAA: Screen at 65. 2. Hypercholesterolemia 3. Vitamin D Deficiency 4. Impaired Fasting Glucose 5. Gout 6. History of surgery brain surgery for right facial paralysis, retained metal plate, 2007. appendectomy 82. Cerebral embolectomy 7/2020. Left eye laser procedure for retinal tear 9/22. 7. Dry eye syndrome Right eye due to mild residual paralysis. 8. Social and personal history finding Tobacco: Former smoker and chewing tobacco. Alcohol: 1 vodka tonic/week, 3 shots. Rec drug: Denies. Currently unemployed due to stroke and pandemic. Was mechanic supervisor. Married. 9. Family medical history Mother: deceased, colon cancer, unknown type of skin cancer. Father: deceased, pancreatitis. Grandparents: PGF - MI. 10. HTN - Hypertension 11. Low Back Pain 12. Neck Pain 13. Herpes zoster with ophthalmic complication April 2019. Treated at optometry. 14. Under care of multiple providers PCP: Neurology: Previously Neurology Center. Currently. Cardiology: . 15. History of cerebrovascular accident 7/2020 Treated with TPA and cerebral embolectomy. Neurology appt 10/13/21- Dr. suspects extension of CVA - MRI, Neurocognitve testing ordered 7/12/22 Cardiology - continue ASA and Plavix. 16. Sensorineural Hearing Loss, Bilateral 17.
