Symptomtext

Family consisting of mom and two sons scheduled to get seasonal immunization at the pharmacy on 11-16-23. 13 year old male had appointments for two shots - one spikevax and one fluzone quad immunization. following second immunization patient briefly passed out and had a brief seizure event. the event only lasted about 10 seconds. mom stated that patient has history of passing out at sight of blood but never with immunizations. patient regained consciousness after about 10 seconds , white in facial color and lips slightly blue. patient stated felt a little nausea upon sitting up. patient was offered water and started to regain color and decreased nausea symptom. patient remained in immunization room for another 30 minutes and then left with mom and brother for some electrolyte drink and to go home. mom was going to follow up with doctor in the morning.

Symptomtext

School Nurse reported VR was anxious, saw reddened blotch on VR's neck and noted the VR was touching the area a lot on when walking the VR to the school-based clinic, before VR was vaccinated. Reddened area was again noted when entering observation, the clinic station that follows vaccination. VR was closely monitored for 30 minutes in the observation station. No change was noted during the 30 minute observation. When the mother was contacted she said that she was not surprised and that she also gets red botches in the same area when anxious.

Symptomtext

Patient states that he noticed hives on the same arm as the vaccination site 2 days after the vaccine was administered. On 10/9/23, patient stated that there is still a small amount of irritation/itchines, but they hives that were scratched opened have started to heal. Patient has no primary care physician at this time so he was unable to consult with a physician.

Symptomtext

There was the mother & 1 of the 2 daughters in the room. I gave the 1st daughter her flu and was about to give mom hers next. PCP came and requested that mom go into another room to talk & delivered the 2nd daughter (this patient). I grabbed her flu and then mom's was still in place as she was getting hers next. I gave both her flu & moms flu instead of the HPV that pt was supposed to have received.
