Symptomtext

Patient inadvertently received full vial of undiluted Covid 19. Medical assistant did not use diluent, drew up full vial of Covid vaccine and administered it to the patient. Patient was notified on 12/13 when error was identified. Mom reported that patient had increased spitting up for 1 week after he received his 6 month vaccines and he was very sleepy for 24 hours after his vaccines. He is back at baseline.

Had flu shot in left arm - 2 days ago First night ,a little red around the area of the injection Then got more and more red on the left arm over the last 48 hours- and now sticking out more and hot and painful to touch. 11/3/23 seen in office and prescribed: cephalexin PO SUSP 250 mg/5 mL; Give 10 mL by mouth 2 times daily for 7 days.

Symptomtext

Patient started coughing within about 5 minutes of getting his shots. Felt like he was going to throw up, some dry heaving. Belly hurt, smear of loose stool . Throat felt tight. Lungs clear prior to shots but diffuse wheezing and mild retractions after. Pulse always brisk. Got 2.5 mg albuterol and 0.5 mg atrovent nebs, 0.15 mg epi pen jr, and 10 ml of liquid benadryl. EMT arrived and transported him Hospital for further managment. Known egg allergy but not anaphylactic. Has had both MMR and flu shots in the past with no problems

Pt. recipient Glaxosmithkline Influenza Qaud Inj P Lot# NT2M9 on 10/24/2023 at PCP office. Pt. subsequently received an additional influenza vaccine (info above) at school flu clinic. Pt. recipient parent neglected to notify school nurse child had received flu vaccine on 10/24/2023. If that had been done the school consent form would have been pulled and the incident would not have occurred.

GREAT AUNT OPENED DOOR ON EXAM ROOM AFTER FLU VACCINE WAS GIVEN, ABOUT 3-5 MINUTES AFTER VACCINE WAS GIVEN, SHE SAID SHE NEEDED A NURSE. GREAT AUNT STATED THAT "PATIENT FELL OUT OF HIS CHAIR, JERKED A COUPLE OF TIMES AND WET HIMSELF." GREAT AUNT WANTED PATIENT SEEN AT THE EMERGENCY ROOM, AMBULANCE WAS CALLED AND PATIENT WENT TO HOSPITAL.

Symptomtext

At 4:35pm administered 2nd dose of HPV9 to left arm, influenza vaccine had already been administered to right arm. Child had a jerky of head back episode right after receiving HPV9. It lasted about 3 to 5 seconds, noticed child got pallor, still sitting in chair, I checked vitals B/P 108/60 pulse 80. Child di not lose consciousness, responded to my questions, alert. Skin remained dry to touch, denied any headache, or dizziness. Continued to assess no shortness of breath or swelling of any kind. B/P 110/60 pulse 82 mom says child takes no medications, has no allergies, just healthy. Refused ambulance and B/P 110/70 pulse 70 before leaving.
