Per patient safety report: A staff member received their annual flu vaccine from Health Clinic immunization staff on the afternoon of 9/27/23. They started to experience tingling and numbness in their tongue, facial swelling under eyes near sinuses and nasal drainage in the evening. The staff member had to sought medical services starting the day of the immunization. She continued to have multiple medical appointments throughout the month to treat symptoms as a result of the vaccine. Per patient: There is a trend every year after receiving the flu shot, I get sick and spend the next 3 months-ish recovering from the response. Starts with flu like symptoms usually the day after then increase to the point of being out or work due to sinus infections, bronchitis, etc. Usually end up taking allergy medications, nasal sprays, inhalers (broncodilaters and steroid) , steroids, antibiotics, breathing treatments, ER/ Urgent care visits, etc. Have been seen by allergy clinic in the past, pulmonology (on-post and off-post) sinus and chest CT's. etc. This is the first year, that I had a "true" allergy response and it happened on the same day. The reactions are getting quicker and more severe. My tongue started tingling and numb feeling. my face started swelling under my eyes (in the sinus area),felt like my eyes were swelling shut and tons of drainage, etc. I had appointments with PCM and urgent care again this year. Same pattern, same symptoms, same meds but more severe. Please help? I received the flu shot on Wednesday afternoon (9-27-2023) and was having issues by the time I got home from work that evening around 6:30ish. I immediately took some Benadryl liquid to help prevent the reaction from getting worse. After trying to self-medicate with over the counter meds and meds I already have, my symptoms continued to worsen causing me to continue to be out of work and had multiple doctor?s appointments between 9-27-2023 and 10-27-2023 including visit to urgent care off post 10-25-2023. My PCM is located at Clinic. He noted the allergy on my profile. I was out of work September 28, Oct 2,4,5,6,11,12,13,16,17,25,26,27. You may review my records if needed for further information. Investigation: Influenza vaccine 9/27/2023 approx 2:50pm. Patient began having symptoms of facial tingling/numbness and eye swelling at approx 6:30pm. Self medicated with Benadryl. Was seen by PCM on 10/02/2023 w/ HPI feeling very congested the past week, weakness, cough, and runny nose. Provider annotated acute upper respiratory infection, squamous blepharitis right upper eyelid, and chronic sinusitis. "This happens every year when she gets the mandatory flu shot. Had the flu shot 4 days ago, woke the next day with chills/fever/cough/slight dyspnea. URI symptoms, w/o fever or sputum problems. She also has chronic sinusitis, for several years after upper molar surgery. Also, needs consult for f/u Ophthalmology, re: chronic blepharitis. " Patient went to off post urgent care 10/25/23 and prescribed prednisone and antibiotics. Seen 10/27/23 by PCM for cough and congestion, requesting Muccinex DM.
