Symptomtext

The patient was halfway home when they started to experience a cold shiver and felt the need to throw up. The patient pulled over on the side of the road and for the next 20 minutes was throwing up. Upon arrival at home, the patient was throwing up 2 hours after vaccination and still experiencing a cold shiver, using two blankets and a coat with no relief. The patient also reported a migraine occuring at this time which they took Tylenol for. They then proceeded to sleep for the rest of the day. The next morning patient reported throwing up had stopped and the headache was still present but alleviating.

Patient was administered a Fluzone HD Vaccine on 10/24/23 per her request. Resident had already received a Fluzone HD vaccine on 9/27/23. Error was related to patient and PCP on 10/15/23. Patient reported on symptoms as sore arm and minor nausea on 10/25/23.

Arthralgia Product administered at inappropriate site

Symptomtext

Patient reported to the pharmacy 11/2/23 that she believes the vaccine was given too high on her left arm. She noticed right away as she could feel the vaccine going into her arm at the time of vaccination. She reported the first couple days immediately following vaccination were very painful and she described the pain as feeling like a "hammer hit" to her shoulder. She said after the first couple days, the pain decreased a little to where it feels like a "half hammer hit", but the pain is still there. She has tried ice, ibuprofen, and tylenol with little relief. She has not seen a doctor for this yet, but she has an appointment with her primary doctor in mid-November and will discuss with the them if the pain does not improve.

Patient received fluzone HD on 9/5/2023. Next day started with hot flashes (fever), Next day no appetite, nausea, chest tightness and diarrhea. Went to Urgent care on Friday 9/8/23. Urgent care sent her to ER. ER admitted her overnight to run test. Discharged on Saturday 9/9/23. Hospital said it may have just been a virus. Covid test was negative.

Patient reports diarrhea starting the morning following vaccination and continuing now for 5 days. She has been on a BRAT diet and her PCP offer testing a stool sample. 9/7/23: Patient reports seeing a "wavy rainbow out of side of left eye". She closed her eyes for a few minutes and it subsided. 9/8/23: Patient reports feeling as if she was "moving through jello" as she went to stand up. She claims she did not experience any dizziness, but as if things were moving in slow motion. She laid down to nap and felt better when she got up. 9/12/23: Patient reports feeling as if she was "moving through jello in slow motion" again. She tried to get up but felt very unsteady, as if she didn't have real control of her legs. She felt better after some rest, and no episodes were felt after.
