Symptomtext

Patient states he had radiating pain (radiculitis) and muscle spasms in left shoulder and neck starting the day after the injection and has gotten worse and has been unable to sleep. Patient symptoms got worse 10/10/23 (day 9 after injection). Patient started taking a medrol dose pack starting 10/10/23. Patient went to orthopedic prescriber 10/12/23 and he was given flexeril and meloxicam. Orthopedic prescriber took xrays and diagnosed patient with arthritis in neck. Patient was unsure this was from the vaccine but is highly encouraged to fill out a form.

Symptomtext

Around 4 hours after vaccination on 9/23/23, the patient began experiencing extreme fatigue and aching in all joints and developed a severe headache. She went to bed early but was unable to sleep that night. The next morning, she felt dizzy and nauseous. She felt an urgency to go to the bathroom but no diarrhea. She also had chills and fever Saturday night 9/23/23 through Sunday and Monday. The chills were not as severe on Monday. She experienced vomiting periodically Sunday 9/24/23 , beginning late morning and continuing through Sunday evening. She feels like her voice is altered currently but does not have a sore throat.

Symptomtext

FLUZONE HD QIV vaccine was administered to a 25 year old patient with no reported adverse event; Initial information received on 22-Sep-2023 regarding an unsolicited valid non-serious case received from a other health professional. This case involves 25 years old male patient who was administered influenza quadrival A-B high dose HV vaccine [Fluzone High-Dose Quadrivalent] with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 22-Sep-2023, the 25 year old patient was administered a standard dose of suspect influenza quadrival A-B high dose HV vaccine (with an unknown formulation) at a dose of 0.7mL once (lot UT8079DA, expiry date- 30-JUN-2024) via intramuscular route in the right arm for Immunization with no reported adverse event (product administered to patient of inappropriate age) (latency- same day). Caller asked guidance on this. Action taken was not applicable. At time of reporting, the outcome was Unknown for the event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
