administered flu shot then walked patient out to the waiting area. a customer asked me a question while the patient sat down. then the patient said she doesn't feel right, said she was lightheaded and seeing stars. she then proceeded to stiffen and convulse and I yelled for the other pharmacist to call 911. patient convulsed for approximately 15-20 seconds. During this time she also lost control of her bladder. said she was having difficulty breathing so an epipen was administered. ems arrived at approximately 405pm. they took her by ambulance to er to monitor her and also bc her pulse ox was only 89 and she was feeling very weak and shaky.

Symptomtext

Patient came into pharmacy on Tuesday, 7 November , 2023 and said that he experienced a sore arm the day after receiving Fluad on 3 November 2023. The patient seemed to be in pain and had difficulty in removing his sweater. The patient's arm was examined, there were no visible rash or swelling on the shoulder at the injection site. Patient indicated that it was painful on top of his shoulder between his neck and shoulder. Patient's right hand was swollen. Patient stated that the pain improved when he took Ibuprofen, he also stated that he was on Prednisone daily as he had a kidney transplant. Patient purchased Diphenhydramine and was going to follow up with his physician and will report back to the pharmacy in 2 days

Patient called in the morning (11/29/23) and came in at around 11:30am to the pharmacy for me to look at her arm. She had a bruise on her Left arm at where the vaccine was given. There was redness and is swollen from below the vaccine site to her near her elbow. Warm to touch. Patient said the Immunizer (Pharmacist) told her that Fluad given on the left arm, but on consent form said the right arm. I will ask the Pharmacist who gave the shots on his next shift to clarify. Therefore, there's a possibility that Comirnaty was given on her left arm because she got one shot in each arm. I advised the patient to go to Urgent Care or see PCP to stop the reaction.

Symptomtext

24 h after the injection, the pain started in the left side of the neck. Then the pain migrated over time to the base of the skull, then behind the left ear and kept moving up to the top of the head, with extreme sensitivity to the touch of the skin, All this over a 5 days period. In addition, I was feeling fatigue. On day 7, I started to experience a swelling of the forehead. I went to see my PCP who thought that I may have shingles provoked by the vaccine. He started me on Valacyclovir. By Wednesday, I woke up with a swelling of my face (the forehead, the eyes and the cheeks). I saw the PCP who discontinued the Valacyclovir and started a treatment with the Methylprednisolone pak and Cephalexin. By the evening, I went to the ER: vital signs were normal, blood work normal. No swelling of the throat, no signs of inflammatio of the throat. I was released with no conclusive diagnosis, but the doctors felt that the symptoms might have been related to the vaccine. Today, Monday 11/28, most of the symptoms have resolved.

The patient's medical history and concomitant medications were not reported. Patient claims she was very tired and arm ached. She said she didn't "feel right" so she went to bed early. She got up in the middle of the night (does not know what time) and tried to stand up. Patient presented with large (4 inch?) red area which was located about 3-4 inches from where the injection was given. Reports no pain or discomfort. Area is not warm to the touch. Had spread some since initial presentation. Due to location unsure if due to vaccine or not.

Indurated arm; Red Arm; Arm started itching; Arm Inflamed; Warm Arm; This spontaneous case was received on 20-Oct-2023 from other health professional (Nurse, Patient) via agency (reference number: SEQW23-03616) and concerned a 71-year-old, female patient. The patient's concurrent conditions included intestinal problems, allergy to penicillin, sulfur and dairy (milk protein). The patient's concomitant medications included Xarelto (Rivaroxaban), Calcium, unspecified vitamins, Singulair (Montelukast sodium) all used for unreported indication and low dose Ciprofloxacin used for intestinal problems. On 18-Oct-2023, the patient was vaccinated with Fluad Quadrivalent (influenza vaccine inact sag 4v; dose: 0.5 ml, route of administration, anatomical location, and indication: not reported). The batch number reported was 371640 with expiry date: 30-Jun-2024. On the same date, five minutes after receiving Fluad Quadrivalent vaccine, the patient had a local reaction and her arm started itching. The rest of the day it started to get inflamed. On 19-Oct-2023, the patient observed that, the arm was red and indurated. On an unspecified date in Oct-2023, the patient's arm was warm. On 20-Oct-2023, the patient's arm was still red however it was slightly less warm. The outcome of the event inflammation localized, induration, erythema of extremities, localized itching was not reported. The patient's medical history was not reported. The patient's concomitant medications included finasteride, atenolol, valsartan, doxazosin and amlodipine, all for unreported indication. On 08-Nov-2023, the patient was vaccinated with Fluad Quadrivalent (influenza vaccine inact sag 4v; dose: 0.5 ml, route of administration: possibly subcutaneous and anatomical location: not reported) as influenza immunization for patients over 65 years of age and older. PATIENT CAME IN TO RECEIVE THEIR COVID VACCINE AND THE FLU VACCINE. TECHNICIAN VERIFIED WITH THE IMMUNIZATION SELECTION TOOL AND ENTERED INTO SYSTEM THE CORRECT VACCINATIONS. THE PHARMACIST ADMINISTERING THE VACCINES HAD GRABBED THE PEDIACTRIC DOSE OF THE COVID INSTEAD OF THE ADULT DOSE. ONE TECHNICIAN WAS SCANNING INTO THE SYSTEM WHEN WE NOTICED THE VACCINE STICKER WAS INCORRECT FOR THE PATIENT, WE IMMEDIATELY NOTIFIED THE PHARMACIST AND THEN HE PROCEEDED TO CALL THE PATIENT AND CDC.
