Symptomtext
Adult Pfizer/BioNTech vaccine 0.3 ml (Grey top for ages 12 and up) was administered instead of pediatric Pfizer/BioNTech vaccine 0.2ml (Orange top for 5 to <12). No adverse effect noted. Error discovered on 03/25/22 (11 days later) when staff were entering administration into State immunization system. Spoke with patient's mom after error discovered and she reported child had sore arm after injection for a day or so that had resolved and no other effects. Mom reported child was doing fine. Child is 63.5 in tall and weighs 160lbs. Pediatric provider recommended patient not receive scheduled booster. Mom agreed.
