Symptomtext

Client contacted and informed me on 7/22/21 that her are was red, itchy and a little swollen. Reminded her about taking Benadryl, cool/warm compress to the site. Did a follow-up with her and stated had noticed lumps under her arm and that she had and appointment to see a doctor concerning the matter. I agreed that would be the best thing to do. Did a follow-up again and stated physician gave medication for the swollen lymph nodes. a follow-up again made and stated the medication helped with the swollen lymph nodes and that her arm was better and the swollen lymph nodes were gone.

Symptomtext

Received a phone call from patient on 04/30/2021 at 0745 am and stated she woke up the day after her shot with a few bumps at the injection site. She stated "thought I just had a bug bite". went to work and asked the nurses to look at her arms and hands. Stated she had hives and took Benadryl and placed cortisone cream at the site on both arms. Stated she did not have to go to the Emergency Room and the hives and itching was the only Signs and symptoms she has had. Patient Stated' the nurses at the facility she works at are keeping a close eye on it and other than that she feels fine." I did reiterate the importance of seeking medical treatment if symptom continue or worsen. She replied with an understanding.

Symptomtext

Child here with mother today for vaccinations. Received MMR, HPAC, and DHI in left anterior thigh. Received VZV, PCV13, and HIB in right anterior thigh. Immediately after receiving vaccines, noted local site reaction, erythematous flat rash, to right thigh. No other reactions noted at this time. Nurse manager notified. Mother opted to take child to local emergency room via personal vehicle. Attempted to contact mother twice x 2 attempts.

On 16-JUN-2021, the patient was vaccinated with an improperly stored dose of varicella virus vaccine live (oka/merck)(VARIVAX) (lot number: T000698, expiration date: 07-JAN-2022; dose and route of administration were not provided). The vaccine was stored in temperature: ? 11.3 degrees Celsius for 15 minutes. There was no previous temperature excursion. Digital data lodger was involved. No adverse event was reported.
