Seizure like phenomena

Symptomtext

Other nurse was alerted to patient experiencing "seizure like" activity in the waiting area at approximately 1430. Per nurse patient appeared lucid and conscious upon her immediate response. Writer arrived for unrelated reason and saw other nurse was already tending to patient. Patient noted to have legs elevated, and a cool towel on head. nurse asked student nurse to get a snack and gatorade while writer was looking for cold compress on arrival. Writer provided nurse with a cold compress and she placed it on patient's chest approximately one minute after arrival. After 2 minutes of lying down with feet elevated patient stated he wanted to sit up in a chair. Writer and nurse assisted to chair. Writer observed patient for approximately 10 minutes. Patient stated he was feeling better. Patient's representative was instructed to have patient wait another 10 minutes in observation area. Both patient and patient's representative verbalized understanding.

pt in office on 10/25/2023 and received 4 vaccines; by evening on day of office visit MOC noticed right leg with warm raised rash to skin at injection site with approximate size of mini orange (cutie); pt did not have any pain or itch at that time; by next morning sx's were still the same; by Thursday evening (10/26/2023) MOC noticed same sx's to leg leg/thigh with approximate size of a quarter; this morning (Friday 10/27/2023) sx's of rash were lighter in presentation and the rash to right leg appeared to have spread

no additional AE information; TE VARIVAX, and PROQUAD that occurred on three separate occasions since 11/27/2023. The caller stated that the vaccines had possibly been administered to multiple patients/given VARIVAX on 27-NOV-2023; This spontaneous report was received from a medical assistant and refers to a 12-month-old patient of unknown gender. The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 27-NOV-2023, the patient was vaccinated with improperly stored (product storage error) dose of varicella virus vaccine live (oka/merck) (VARIVAX) injection, lot # reported and confirmed as: X006982, expiration date reported and confirmed as: 07-MAR-2025, administered for prophylaxis (exact dose, site and route of administration were not provided). The stability evaluation revealed that excursion was supported and the vaccine had not been exposed to any prior excursions. Fall Lip injury

Symptomtext

Child received vaccination and appeared to be in good health. After vaccination, child stood up. When nurse looked back at the child she was on the ground. The fall resulted in a minor injury, including an abrasion and small cut on the bottom lip. Child was responsive immediately after fall. Able to follow commands, sit and stand. BP 110/60 5 minutes after fall.

The patient's medical history, concurrent conditions and concomitant therapies were not reported. On 26-SEP-2023, the patient was inadvertently vaccinated with varicella virus vaccine live (oka/merck) (VARIVAX) injection, 0.5 milliliter, administered intramuscularly for prophylaxis (lot number X006982 has been verified to be a valid lot number, expiration date reported and validated as 07-MAR-2025) (anatomical location was not provided) instead of diphtheria toxoid (+) hepatitis b virus vaccine rhbsag (yeast) (+) hib conj vaccine (ompc) (+) pertussis acellular 5-component vaccine (+) poliovirus vaccine inactivated (vero) (+) tetanus toxoid (V419) (VAXELIS) injection for prophylaxis (strength, dose description, lot #, expiry date, and route of administration were not provided) (product administered to patient of inappropriate age) (wrong product administered).
