Acute hypoxic respiratory failure: with post-infective bronchospasm due to recent COVID -remains on room air with satisfactory o2 sats - nebulizer set up for home 3. AKI, hyponatremia: -resolved 4. Pulmonary nodules: -visualized on CT chest, noncalcified granulomas vs metastatic disease -pulm consulted 5. Hypokalemia; replacement given 5. Other chronic medical conditions: History of asthma, chronic back pain, history of breast cancer, chronic pain, cognitive developmental delay, CAD, history DVT, depression, history factor V Leiden, hyperlipidemia, hypertension, hypothyroidism, morbid obesity, OSA, port wine stain to face, history of PE, history of renal cell carcinoma, history of seizure, history of stroke, Sturge-Weber disease Patient was initially admitted to the hospital following COVID symptoms starting February 20. Patient was discharged on the 27th. She returned to the ER on 3 March return to the ED was found to have pneumonia initiated on Vanco and Zosyn in ED. Patient was further evaluated by pulmonary who found on her CT PE study bronchopneumonia. Discussed nebulizer treatments. No further treatment necessary for COVID from the 20th We will discharge to home with a course of doxycycline and have patient follow-up outpatient with her primary care provider; patient agreeable to plan

Patient began having symptoms of cough congestion and shortness of breath and early August and was started on antibiotic therapy at that time. This was continued for approximately week to 10 days and then extended by his primary care provider. He continue his antibiotics almost until about a week ago and had continued improvement in symptoms. Recent imaging had shown ongoing pneumonitis in this region but improvement. However this morning he woke up complaining of severe shortness of breath and dyspnea on exertion. He had a productive cough but it was mostly clear and white in nature. No hematemesis or hemoptysis. No symptoms of chest pain or chest discomfort. No complaints of peripheral edema. He has not started radiation or chemotherapy as of yet. He stopped smoking 3 weeks ago. He is also increasing more somnolent this morning as well. When EMS arrived his oxygen saturation was 82% on 3 L and he was started on nonrebreather mask prior to coming into the emergency room. No symptoms of palpitations. He has been dizzy with arising from recumbency. Baseline oxygen is 2 to 3 L per nasal cannula. In the emergency room his blood pressure was 98/61 pulse rate 91 respiratory rate 19 oxygen saturation was 90% on 10 L nonrebreather mask temperature 36.4. Serology: White count 13,700 with slight left shift hemoglobin 13 hematocrit 40.8 and platelet count of 362,000. Sodium 138 potassium 4.5 chloride of 102 bicarb 27 BUN 20 creatinine 1.2 and a blood sugar of 215. Calcium 9.5 troponin of 20 serum lactate 0.9. Arterial blood gas pH of 7.28/PCO2 60/PO2 73 with O2 sat of 94%. COVID PCR negative. CTA of the chest showed no obvious pulmonary embolism. There is evidence of a right perihilar mass which encases the right lower lobe segmental pulmonary artery with mild narrowing. Opacification of the right primary bronchus and right lower lobe segmental bronchus which could represent secretions or invasion by neoplasm. There is a new airspace infiltrate in the right lower lobe posterior basal segment likely postobstructive pneumonia. Patient received Moderna COVID vaccine on 1/14/2 (lot # 024M20A), 2/11/21 (lot # 024M20A), and 12/30/21 (lot # 013H21B). On 8/24/22, patient admitted to our inpatient facility with acute hypoxemic respiratory failure due to COVID-19 pneumonia. Patient transferred to med/surg unit on 8/26/22 and as of today (9/2/22), patient is still admitted in the med/surg unit.

She got her vaccine, she was driving home at 11:30 at night and all of a sudden her car started going into the wrong land and almost hit a bridge and determined that she could not drive. She had just brought her sister-in-law home, was tired, and was hungry and tried to eat. She realized that she was feeling very tired and tried to get up and realized she had a hard time moving, and used furniture. She called her daughter who took her to the hospital to the Medical Center, who did a CAT scan immediately and was a brain stem stroke. She was there for 2 1/2 weeks, and then transferred to Rehab hospital for about 3 weeks, and has been doing out-patient. She fell on 10/3/22 and broke her hip and leg. She is now on Amlodipine, Atorvastatin, Lisinopril, Xarelto. She can use a walker, but does not have her balance back and cannot use a cane. She also has a knee problem and needs a replacement, but can't due it yet. She had a hip replacement after she fell and broke her hip and her fibula. She still cannot walk mainly because of the stroke. She was very healthy prior to the vaccine, took care of other people and had no issues and was still working, and since the vaccine she has not been able to go anywhere and has to do everything on line, and not able to work now. Was in Hospital initial for about 2 1/2 weeks, and then Hospital for Rehab for 3 weeks, and then returned for another 2 1/2 weeks, and then now out patient therapy. This destroyed her ability to take care of herself, and doing everything she can to get back to where she was and hopes that she will be able to. She cannot drive anymore and is scared to after having the stroke.

Fainting spell; Strongly positive antigen test; Weakness after going to toilet; Extreme dizziness; Return of fever (99.5-99.9F); This spontaneous case was reported by a consumer and describes the occurrence of SYNCOPE (Fainting spell) in a patient of an unknown age and gender who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch no. 013H21B) for COVID-19 vaccination. The occurrence of additional non-serious events is detailed below. Concurrent medical conditions included Anemia, Alpha thalassaemia and Drug allergy (Drug was not specified). Concomitant products included FAMOTIDINE from 13-May-2022 to an unknown date for an unknown indication. On 30-Dec-2021, the patient received third dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 26-May-2022, the patient experienced SYNCOPE (Fainting spell) (seriousness criterion medically significant), COVID-19 (Strongly positive antigen test), ASTHENIA (Weakness after going to toilet), DIZZINESS (Extreme dizziness) and PYREXIA (Return of fever (99.5-99.9F)). The patient was treated with NIRMATRELVIR, RITONAVIR (PAXLOVID) from 18-May-2022 to 22-May-2022 for COVID-19 treatment, at a dose of 1 dosage form. At the time of the report, SYNCOPE (Fainting spell), COVID-19 (Strongly positive antigen test), ASTHENIA (Weakness after going to toilet), DIZZINESS (Extreme dizziness) and PYREXIA (Return of fever (99.5-99.9F)) was resolving. DIAGNOSTIC RESULTS (normal ranges are provided in parenthesis if available): On 26-May-2022, Body temperature: 99.5 - 99.9 f (High) 99.5 - 99.9 F. On 26-May-2022, SARS-CoV-2 test: strongly positive antigen test. (Positive) Strongly positive antigen test.. The patient took other medications/products within 2 weeks of receiving COVID-19 treatment. Patient stated return of fever (99.5--99.9F) on 26-May-2022 (d4 after stopping paxlovid, d10 post start of symptoms). Patient went to ER for hydration. Patient received IV fluids as treatment for the adverse event. CC: This spontaneous case concerns a patient of unknown age and gender, with a relevant history of anemia and alpha thalassemia, who experienced the unexpected non-serious adverse event of special interest COVID-19 and the serious (medically significant) event of Syncope, that occurred 147 days after receiving mRNA-1273 vaccine as third dose in the COVID-19 vaccination series. Patient stated return of fever (99.5--99.9F) on the 4th day after stopping Paxlovid and on the 10th day after initial occurrence of symptoms, and experienced extreme weakness, fainting spell and dizziness. Patient was brought to ER for hydration with IV fluids. Outcome of the event was reported as resolving. Patient completed 5 days of treatment with Paxlovid. 5 mins after receiving dose pt fainted. 911 was called. slowly brought pt to the floor. attempted to arouse pt, was unresponsive for 10-15 seconds. pt was feeling much better by the time ems arrived in approximately 15 mins. vitals were stable. pt agreed with ems personnel he needed no additional treatment.

Body aches and fever caused me to be unable to open my eyes. The next day I started to feel that my armpit became swollen for about 4 days long. On the 27th, I was unable to close my left eye, and on the 28th my mouth was numbed on the left side. I went to the ER and they stated I had high blood pressure and was given medicine to treat it. Treated as a stroke however, MRI and CT Scans showed no stroke symptoms and stated that I have Bell's Palsy. I still have paralysis and it will take a few months to recover from it. No more swelling in arm areas.

Pt came in for Moderna booster (half dose) after about 3 mins of received shot Pt complained of dizziness and blurred vision. Took Pt back into vaccine room pt became pale, complained also about nausea, paleness caused shallow breathing, numbness of hands. on the form pt did not say that she had rxn to prev covid vaccine (or any other vaccine) in general, also when I asked open ended question pt did not say she has reaction before- in the room pt mom and pt mentioned that these same exact sx happened after 2nd dose but this dose even worse. pt did not want to have anything to eat or drink. pt said this happened to her after 2nd shot as well, but it is worse this time. we fanned her and provided icepacks to cool her down. asked pt if she wanted me to call 911 and she said yes, told them that they may be charged and mother said that it is ok b/c they have insurance. emt came in and took vitals, they said that everything is normal and that they can go to hospital if she wants. they said she might be scared of vaccine and had vasoreactive reaction. pt felt much better and was able to walk on her own. pt mother said that she will not be taking her to ER.

Patient lost consciousness almost immediately after receiving immunization. Symptoms: Loss of Consciousness, Low B/P, Paleness, Sweating, Cold/Clammy skin, Nausea, Dizziness, Loss of vision. Starting in June of 2021 I had developed a tick like rash on my left arm. It was not a tick. That summer I had pins and needs in my hands. I received the Moderna booster on December 28, 2021. The rashes developed into MRSA and HSV1 on my upper lip. Following that MRSA event, I developed nerve pain in the back of my left thigh which traveled to my bottom buttocks both sides. I had nerve pain in my face which has continued. I have seen a neurologist who has done test after test and all have been normal with the exception of the past MRSA infection. We have no explanation for my nerve damage.

a week after the shot, started using a wrist ECG and it displayed over many unusual rhythms. At the time, there were so many oddities, I couldn't make sense of them. This continued until May, when it was bad enough I went to above hospital for 'palpatations'. They recorded many PAC, PVC and some triplets. My skin symptoms started with red bumpy rash on my abdomen. The front of my abdomen and sides and didn't notice it anywhere else. I treated it myself with clindamycin liquid. That's what was given me to treat skin problems in the past. I used that and it cleared up pretty quickly within a couple weeks. I also happened to have extremely red itchy eyes that were matted in the morning and goopy during the day. I treated that with a warm moist compress. I used a hypochlorous acid to clean my eye lids, OTC lid and lash cleanser that my doctor had recommended. The facial symptoms started towards the end of February 2022. Again red spots on my face that gradually increased and each day there would be an additional accumulation of red spots. they didn't itch and they weren't really annoying. some of them were papular so I tried to use the clindamycin on those and really kept getting more and more on my face and are going down my neck on each side of my throat and the back of my neck as well. general itchiness all over body, but there was no rash in other places. scalp was really itchy as well, but not on my face. Another odd symptom I had was- I did have a PCR test and my left nostril internally did not heal well. I had some bleeding with that and it is just now finally getting better. as my face started to get better, the inside of my nose also started to get better. it just happened to be the PCR test irritated it. I'm unsure. One more thing to mention, I've noticed an increase in migraines, even with the first and second dose. and Since the booster, I've had to use my migraine medication more frequently. I saw the dermatologist for the facial breakout on March 14, 2022. She gave me another cleanser to use, FINACEA 15% foam and recommended that I use that twice daily. It would cause some burning and stinging which it did... but it definitely helps whatever the rash was to go away. she thought it was Rosacea which I have not had before. I had my follow up with her yesterday and she recommended to continue to using this cleanser twice a day, but my face is clear now- the way it used to be.

in March/April 2021, Due to continued breathing issues with breathing, my PCM referred me to a Pulmonologist, Dr. I was placed on a course of Eliquis for 6 months Again in Jan 2022, i was experiencing heaviness and pain in my chest. I contacted Dr. to see if it was possible for blood clots to returned. He stated it low probability but order blood tests to be sure. over the course of the day, the pain got worse so I went to the ER Currently on Eliquis, indefinitely.

Two weeks after getting vaccinated I began to hear loud hissing in my ears. This noise has gotten worse and now it is loud enough to wake me up from a sound sleep. Got my booster on Thursday, around 4pm site was sore and tender, woke up. Around 2am left arm swollen, difficulty moving and lifting. Patient stated that after getting the moderna booster, she felt sick for 5 days (chills,muscle pains, general under the weather feeling). Patient reported falling asleep on the job after getting the vaccine (patient stated she works as a taxi driver and fell asleep while parked outside a client's house after dropping them off about a day or two after getting the booster). Patient also reported having a bad reaction to beer/alcohol. After she got the vaccine, she went out with some friends and had a sip of a friend's beer. Patient then reports getting violently sick to her stomach and throwing up. She stated that she was required to take several days off from work due to the fatigue, chills, muscle weakness, and nausea/vomiting. Patient stated that she feels that she needs to take extra breathes that were not needed prior to administration of the booster. Patient reiterated that when she tried drinking some alcohol that it "tasted like poison" and it caused the patient to experience diarrhea and vomiting. Additionally, she mentions that she did once again vomit this morning. When I asked the patient if she had been experiencing any of the aforementioned symptoms with the first 2 doses of the Moderna vaccine, she said that it was not to this extent and she may have been exhibiting symptoms from previously having COVID in January 2021 (as she self-reported and has noted in EHR). Patient does not feel that she has fully recovered from being positive with COVID from January 2021, but she feels strongly that her current symptoms are due to being allergic to the vaccine/vaccine components.

I went through Menopause and hadn?t had a hot flash or night sweats in over 8 months. After getting the booster on 1/19/22, they started up again a few days later. I?m getting them almost daily and nightly. Worse than when I went through it the first time.

DIAPHORESIS, DYSPNEA

My appointment was set for 1:50pm. After 3 minutes: a quick spirt of dizziness but was sudden. After 5 minutes: chest pain and tightness on the left side of my chest that felt like it was coming from my heart. After 10 minutes: pain from deltoid and across pectoralis muscle groups. After 12 minutes: pain on the left side of my breast - a pressure/heaviness. After 20 minutes: migraine and dry mouth, told the nursing staff about my reactions. 2 hours after shot: nauseous, constant pressure on left breast, along with arm soreness. 2-3 hours after shot: fatigue, nauseous. 3-4 hours after shot: left calf pain, pain in calf is NOT an ongoing medical issue. Over night: still had dry mouth regardless of the amount of water I drank, upset stomach/diarrhea, chills, and fever. Next morning and 3 days after: dry mouth, fatigue, bloody noses (NOT typical), nauseous, soreness, breast tenderness, no appetite, fever, chills, and cloudiness. These symptoms are not common for my person.

Patient was experiencing hives 10:43 am; PCP notified by LPN Patient was evaluated by PCP , found to have decreasing o2 saturation, sob, wheezing and hives. Patient was treated with stat, 50mg po qd of benadryl, 80mg IM methylprednisolone and 1 inhalation of albuterol. Pateint suffers from asthma, but denies having any wheezing prior to receiving the injection. AFter all treatments, pt.'s symptoms improved.

The day of my vaccination, my arm was sore by 3PM, and I had a pretty bad headache and felt fatigue by 9PM. My left arm was sore. By bedtime, it was achy and even more sore. He also used a cream on his left arm which has helped with the pain. He had not used the cream on his legs. He also noticed spots on his right buttocks cheek and left leg that had pus in them. He scraped those areas off and put a band aid on them with antibiotic cream. The skin symptoms continued up until the time of report.
